Description
SCREENING INFORMATION REQUEST (SIR) / REQUEST FOR PROPOSAL (RFP) No. DTFAAC-12-R-03678 DOPPLER VOR SIDE BAND 2 ft. HIGH PLASTIC DOME ANTENNA SHELTERS, NSN 5985-00-198-4906 MANUFACTURED IN ACCORDANCE WITH FAA SPECIFICATION FAAD-F-1365B AND DRAWING NUMBER D5701. 1ST ARTICLE WILL BE EXPECTED. The Federal Aviation Administration (FAA) has a requirement for Doppler VOR Side Band 2 ft. high plastic dome antenna shelters, NSN 5985-00-198-4906 manufactured in accordance with FAA Specification FAAD-F-1365B and Drawing D5701. First Article will be expected. The FAA anticipates that a 1 year Indefinite-Delivery/Requirement type contract with firm-fixed pricing arrangements with four 1- year options will be awarded as a result of this solicitation. This procurement is restricted to competition and 100% set-aside to small business. The principle North American Industry Classification System (NAICS) code anticipated for this effort is 326199: All Other Plastics Product Manufacturing, with a Small Business Size Standard of 500. When responding to this Screening Information Request/Request For Offer (SIR/RFO), each offeror is required to submit a written proposal in accordance with instructions provided in the Screening Information Request (SIR) Section L and evaluation criteria in Section M. All interested sources should first attempt to download the attached SIR document(s). Please proceed to the FAA Contract Opportunity's website at faaco.faa.gov to view all attachments as they may not transfer correctly to other websites. If downloading is not possible, all requests for copies of the SIR document(s) must be made in writing to the attention of Lori Lemons, Contract Specialist (AMQ-210), via e-mail at lori.lemons@faa.gov or FAX number 405-954-3111. NO PHONE CALLS. ALL QUESTIONS MUST BE IN WRITING AND EITHER MAILED, FAXED OR EMAILED TO THE CONTRACT SPECIALIST. Any questions submitted for consideration must be received 10 days prior to the solicitation due date. Proposals from unsuccessful offerors will NOT be returned to the offeror. Only one proposal from each offeror shall be considered. Offerors submitting proposals by hand-delivery will need to consider allowing sufficient time to process through the security procedures in place at MMAC. Overnight delivery of responses to this SIR/RFO may also be impacted if not sent in sufficient time to allow for the special mail-handling procedures in place at MMAC (Mike Monroney Aeronautical Center). In accordance with SIR Section L provision 3.2.2.3-14 entitled, "Late Submissions, Modifications, and Withdrawals of Submittals," any proposals received after the due time and date of 3:00 p.m. CT, June 05, 2012 will not be considered. In addition, if all requested information is not furnished in the offeror's proposal, the offeror's proposal may be determined to be non-responsive and ineligible for consideration for contract award. This SIR/RFO is not to be construed as a contract or a commitment of any kind. The Government shall not be liable for payment of any costs attributed to the preparation and submission of requested proposal documentation. Prior to award of a government contract, it is mandated that the contractor must be registered with the Central Contractor Registration, www.ccr.gov. Interested firms are required to submit the attached Business Declaration form and a copy of their SBA 8(a) certification letter, if applicable with their proposal.
