Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This RFQ P12PS22654 and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-58. (iii) This solicitation is a total small business set-aside, NAICS 621111, Offices of Physicians (except Mental Health Specialists) with a small business size standard of $10.0 million dollars. The National Park Service encourages the participation of small, disadvantaged, and women-owned businesses. (iv) The Government intends to issue a firm-fixed price purchase order to the responsible offeror whose total offer represents the lowest price. Gateway National Recreation Area, Sandy Hook Unit, Fort Hancock, NJ Monmouth County, is seeking a health practitioner having the degree of M.D. (Doctor of Medicine) primarily engaged in the independent practice of general or specialized medicine. They can operate private or group practices in their own offices (e.g., centers, clinics) or in facilities of others, such as hospitals or medical centers. General physician services are required to perform physical/medical examinations for newly hired seasonal surf/lifeguards and maintenance workers and permanent employees for Sandy Hook Unit. (v) LOCATIONS: 1.The cited medical examination will be performed at the medical practitioner's place of business. (vi) SCHEDULE of SUBMISSIONS: One (1) invoice must be submitted after successful delivery and acceptance. (vii)SCHEDULE: The period of performance will run principally from time of award through June 2012. However, medical exams may be needed year round. (viii)DOCUMENTS TO BE INCLUDED IN QUOTE: 1.A firm fixed price quote, provided on the attached bid schedule. 2.Three (3) references of the proposed services within the past five years 3.Name, phone number, and address of your point of contact. 4.Proof of Central Contractor Registration (CCR) (ix) The Government intends to issue a firm-fixed price contract to the responsible offeror whose total offer represents the lowest price. (x) A quotation submitted in response to this solicitation is not an offer and, consequently cannot be accepted by the Government to form a binding contract. The purchase order is an offer by the Government to the vendor to buy supplies and/or services upon specified terms and conditions. A contract is established when the vendor accepts the offer. (xii)The official combined synopsis/solicitation is posted at the FedBizOpps website: (www.fbo.gov) and http://ideasec.nbc.gov. In order to receive the award, vendor must be registered at http://www.ccr.gov. Be prepared with your company's DUNS number (available from Dun and Bradstreet at 1-888-546-0024) and your Federal Tax Identification Number (TIN). Amendments to the solicitation will be posted in the same manner as the solicitation; it is your responsibility to monitor the website. The government reserves the right to cancel this solicitation. (xiii)Offers must be submitted via email to NPNH_Contracting_Bid_Proposal@nps.gov, to the attention of Linda Richardson, with a courtesy copy sent to linda_richardson@nps.gov. The due date and time is 2:00pm ET on Tuesday, May 29, 2012. (xiv)STATEMENT OF WORK FOR MEDICAL EXAMINATION: For the newly hired Lifeguard/Surf guard and Maintenance Worker Personnel at Gateway National Recreation Area, Sandy Hook Unit, Fort Hancock, NJ Monmouth County. 1.0Background Gateway National Recreation Area hires Surf-Lifeguards for the protection of visitors during the spring/summer season. These employees are required to successfully meet the physically rigorous and hazardous challenges of lifeguard work at an oceanfront. This can involve, for example, first running, then diving, into turbulent surf; swimming against the powerful force of surging tides and alongshore currents to rescue swimmers in distress or in danger of drowning; and then, without any interruption, administering Cardiopulmonary resuscitation nonstop for a number of minutes. As a consequence, Gateway National Recreation Area requires a medical doctor to certify that a Surf-Lifeguard and Maintenance worker candidate meets all the physical-fitness requirements (seen in the attached OPM Optional Form 178 - Certificate of Medical Examination) before it will permit that candidate to be certified as eligible to enter on duty. Note: The title "Lifeguard" falls under the category of "emergency services" employees who are subject to RM-57, NPS Director's Order # 57: Occupational Medical Standards, Health and Fitness. (xv)2.0 Objectives The objective of the cited medical examination is to determine which Surf-Lifeguard candidates meet all the physical-fitness requirements identified in the attached OPM Optional Form 178 - Certificate of Medical Examination for Surf-Lifeguard and maintenance worker work, and which do not. Those who meet all the requirements will be certified as eligible to enter on duty. Those who do not meet all the requirements will be certified as ineligible. (xvi)3.0 Scope Under this contract, Gateway National Recreation Area expects to have its roughly 85 Surf-Lifeguard and roughly 15 Maintenance Worker candidates seeking employment at the Sandy Hook Unit be medically evaluated to determine whether each of them meets all the physical requirements identified in the attached OPM Optional Form 178 - Certificate of Medical Examination for Surf-Lifeguard or Maintenance Worker work, or does not meet all of them. (xvii)4.0 Tasks The contractor will perform the above-cited work in accordance with the best practices currently applicable to professional medical exams of the above sort in the states of New Jersey or New York. (xviii)5.0 Delivery Within seven (7) calendar days of the medical examination in question, the contractor must deliver the cited OPM Optional Form 178 - Certificate of Medical Examination, completed and signed by the examining physician, for each Surf-Lifeguard and Maintenance Worker candidates. The form shall be returned to the employing agency in the pre-paid/pre-addressed "Confidential-Medical" envelope provided. (xix)6.0 Government-Furnished Property No Government-Furnished Property will be used to perform the cited medical exam. (xx)7.0 Security The medical practitioner is responsible to insure Section A of the Optional Form 178 is completed and signed by the Surf-Lifeguard or Maintenance candidate to release the results of the cited medical exam to the Government for purposes of eligibility for employment. (xxi)8.0 Place of Performance The cited medical exam will be performed at the medical practitioner's business place. (xxii)9.0 Period of Performance The period of performance will run principally from time of award to June 2012. However, medical exams may be needed year round.
