- The Chemical Materials Agency (CMA) is conducting a market survey to identify small business with demonstrated capabilities to institute and manage an employee wellness program. CMA is seeking small business with experience in administrative, health management and clinical expertise to establish a health and wellness program to promote health, prevent disease and manage known conditions among its employees. Key Capabilities Desired: The ability to establish a health and wellness initiative based on the clinical science and evidence base of preventive medicine. To include the following; a. Integrate primary prevention strategies to promote healthy and productive lifestyles, to include health promotion and education, immunizations, lifestyle management, and other wellness services and activities. b. Integrate secondary prevention strategies to promote early detection of diseases and conditions, to include health risk appraisals, biometric screenings, and laboratory tests. c. Integrate tertiary prevention strategies to promote better management of known conditions, to include medication compliance, prevention of complications, and improved access to critical interventions. d. Integrate technology tools and innovative approaches that extend beyond traditional disease management and health promotion programs, to include alternative medicine, spirituality care, intensive case management, or other modalities of care. e. Manage a national health and wellness initiative with central administration, a mobile provider team, and an appropriate technology platform to manage a geographically dispersed population. f. Evaluate the health and financial impact of the agency-wide health and wellness initiative The Government intends to establish a US Army Chemical Materials Agency Health and Wellness Office in Aberdeen Proving Ground, Maryland, consisting of a Program Director, Services Coordinator, and Evaluation Specialist. The Services Coordinator shall be full-time and remain on-site at Aberdeen Proving Ground. The Program Director and Evaluation Expert may be on-site or at contractor site during the period of performance. US Army Chemical Materials Agency has interest in the following: a.The necessary staffing, expertise, educational resources, and other requirements to implement agency wide primary, secondary and tertiary prevention strategies. Primary, secondary and tertiary prevention strategies targeted at health promotion, health education and lifestyle management. b. Integration of innovative models for health and wellness Services and strategies designed to modify behaviors and create an organizational culture of health and wellness. The US Army Chemical Materials Agency seeks to develop an evidence-based and comprehensive health and wellness strategic plan for its workforce, to include. a. Establishment of an objective technical advisory panel consisting of preventive medicine experts to review and advise on clinical practices within the agency health and wellness initiative, as well as to review the agency health and wellness strategic plan and program evaluation reports and provide recommendations and inputs. (Objective reviews of clinical practices and program evaluation measures is required, and it is expected that integration of a technical advisory panel will serve this independent review function.) b. Measurement of health-related productivity losses. Company Credentials: Health and Wellness Initiative Program Manager. The program manager must possess expertise about preventive medicine practices, experience in managing national health programs,knowledge about the mission of the U.S. Army Chemical Materials Agency, and the ability to present findings to senior military and civilian leaders. Requirements include: a. A Board-certified physician and Fellow of the American College of Preventive Medicine b. Additional education and training in management and population/public health. c. At least 10 years of experience managing federal and/or military health programs. d. At least 2 years of experience working in the chemical, biological, radiological, and nuclear (CBRN) space as a health care provider and health program administrator. e. Demonstrated experience in presenting to senior military and civilian agency leaders. f. Geographical proximity within 120 miles of Aberdeen Proving Ground, Maryland. Health and Wellness Service Coordinator. The health and wellness service coordinator must have the following credentials: a. Registered nurse; other nursing professional; social worker; or licensed counselor. b. At least 8 years of experience managing federal and/or military health programs. c. Demonstrated experience in wellness services, preventive medicine, or occupational health. d. Geographical proximity within 120 miles of Aberdeen Proving Ground, Maryland. This is a request for information ONLY. NO CONTRACT AWARD WILL BE MADE FROM THIS PUBLICATION. There will not be any opportunities for questions or answers related to this RFI. WHITE PAPER PREPARATION AND SUBMISSION: All domestic small businesses are encouraged to respond to this RFI by submitting a white paper. If already involved in a contract with the Department of Defense, include that contract number. Also, the respondent should state if it is a small business and if so if it is considered small disadvantaged (8a). The white paper shall be limited to twenty (20) pages (8.5inches x 11inches) of English text. It shall be single-spaced, 12 font size with 1 inch left, right, top and bottom margins. Any proprietary data that is intended only for evaluation purposes by the Government must be identified and appropriately marked. See Federal Acquisition Regulation (FAR) 3.104-4; 52.215-1(e). The respondent must also identify all technical data contained in the white paper that is to be treated by the Government as subject to restrictive markings. In the absence of such identification, the Government will assume to have unlimited rights to all technical data in the white paper. The white paper shall be submitted in Portable Document Format (pdf) or Microsoft Word format and include an electronic copy on a Windows compatible CD-ROM. Respondents are responsible to ensure electronic copies on CD-ROM are virus free.
