- Attachment C - NGA Supplied Test Form Attachment B - Technical Evaluation Plan Attachment A - Statement of Work Request for Proposal Pages 7 to 43. Request for Proposal Pages 1 to 6. **URGENT NOTICE - Part of the government's technical evaluation of proposals for this requirement will include an onsite technical field test at Holloman Air Force Base, New Mexico. See Attachment B, Technical Evaluation Plan, for additional details. Vendors will be responsible for the costs of transporting personnel and equipment to New Mexico, and the test is expected to take place on either 6, 7, or 8 June 2012. Vendors will be required to complete and submit Visitor Forms that include the applicable personal information for the personnel that will take part in the onsite technical field test. These forms must be submitted to the government as soon as possible after the publication date of this Request For Proposal. All questions related to this process and the required forms shall be directed to Mr. Scott Hardin at scott.j.hardin@nga.mil.** This is a combined synopsis/solicitation for commercial items (Solicitation No. HM157512T0010, and three additional attachments are included). This announcement constitutes the only solicitation, and proposals are being requested. The North American Industry Classification System (NAICS) code for this acquisition is 334511. The NGA plans to issue a firm fixed priced Requirements type contract as a result of this Request for Proposal. Please reference the attachments for item descriptions, quantities, and periods of performance for each individual contract line item number. Delivery is FOB Destination, and delivery shall be made to NGA, 3838 Vogel Road, Arnold MO 63010.
