Description
Woodstock Job Corps Center On-site services Adams and Associates, Inc., contracting for the U.S. Department of Labor, for the operation of the Woodstock Job Corps Center, a 505-student residential training facility located at 10900 Old Court Road, Woodstock, Maryland 21163. We are currently soliciting a request for written bids for an on-site Dental Hygienist. We are looking for a Dental Hygienist to perform 10 hours per week for 48 weeks. The period of performance for this subcontract is from August 1, 2012 through July 31, 2013 with a 1 year option. Option years will be awarded at the discretion of Adams and Associates. Written quotes should include cost price per hour for the following time periods. August 1, 2012 through July 31, 2013 and August 1, 2013 through July 31, 2014. "THIS IS A SUBCONTRACTING OPPORTUNITY" SCOPE OF SERVICES The undersigned Dental Hygienist agrees to provide students of the Woodstock Job Corps Center with the services stated herein. Such services will be provided as authorized by the Center Director or his/her representative. The Center and the Dental Hygienist hereby agree that the services to be provided will include, but will not be limited to those oral health and wellness services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510; the Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.10, R2; and applicable Technical Assistance Guides). The Dental Hygienist shall: •A. Provide dental hygienic services for a maximum of ten (10) hours per week, excluding student holidays and breaks. Any additional hours must be approved in advance by the Center Director. The services will be provided at the facilities located at the Center Wellness Services Clinic. The maximum weeks of service will not exceed 48 weeks per year. Services are not required during DOL-mandated Center shutdowns that occur during the summer (2 weeks in July) and winter (2 weeks in December). The Dental Hygienist will not be paid during this period, unless the Dental Hygienist is required by the Center to provide services or attend Center-mandated training during this shutdown. The Center will notify the Dental Hygienist of planned shutdowns as early as practicable. •B. Provide a dental hygiene program, including but not limited to, routine, personal dental hygiene services for all students from admission until separation from the Job Corps program which includes preventive and corrective care, as defined by PRH, Chapter 6. Services shall include, but are not limited to: 1. Providing dental care instruction to students through the dental health module of the Health Education Program on an individual basis. 2. Consulting with Center Wellness staff, Counselors, Residential Advisors, Instructors and other appropriate staff concerning the ongoing dental health problems and needs of students. 3. Developing and/or assuring compliance with infection control policies and procedures. 4. Ensuring the maintenance, confidentiality and safeguarding of all student dental records. •C. Provide direct clinical services to students, including: 1. Providing preventive dental health education which will promote good oral hygiene techniques (e.g., tooth brushing, flossing and rinsing instructions). 2. Establishing priorities for providing comprehensive dental care, based upon diagnoses and within program constraints. The Dental Hygienist agrees to adhere to the policies, procedures, and guidelines established by Job Corps. It is understood and agreed that the services provided by the Dental Hygienist are subject to monitoring and review by the Department of Labor. OBLIGATIONS OF THE DENTAL HYGIENIST In the performance of the above duties and responsibilities, the Dental Hygienist: A. Agrees to maintain malpractice insurance to cover the professional services to students, with limits consistent with the medical service industry, but in no case less than $1 million per occurrence. B. Agrees to make no public statements regarding its activities under this Subcontract, or to disclose any information regarding Job Corps students or Job Corps activities without prior written approval of the Center Director or his/her designated representative. Unauthorized release of student records is a federal crime punishable by fine and/or imprisonment. C. Shall be responsible for finding and arranging for a professionally qualified replacement/substitute during temporary periods of absence of more than one calendar week. D. Shall be personally and financially liable for the cost of any patient referral outside the Center without prior written approval of the Center Director. E. Certifies its intent to comply with all equal employment opportunity practices and affirmative action policies as required by law and agrees not to discriminate against any student, employee and/or contractor because of race, religion, color, gender, age or national origin. F. Shall provide current copies of all professional licenses, certifications and professional liability insurance documents required by the Center. G. Agrees that all research projects and/or publications relating to Job Corps health activities will first be cleared through the National Office of Job Corps, Washington, D.C. OBLIGATIONS OF THE CENTER To support the Dental Hygienist in the performance of its duties under this Subcontract, the Center shall: A. Provide an on-Center office area and required non-expendable dental equipment (to include hand pieces, air scalars, cold sterilization, sponge containers, sterilization tongs, pans and hand scalars) and expendable supplies (to include toothbrushes, toothpaste, dental floss and mouthwash) to effectively operate the Dental Hygienic program. B. Provide all USDOL Job Corps guidelines, handbooks, etc., required to operate the Job Corps Wellness Services Program.. All written quotes should be submitted to Denora Brown, Purchasing Agent no later than COB 5pm EST on Friday, June 8, 2012.
