Description
This is not a Pre-solicitation or Solicitation announcement. This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential businesses to include small business sources and their size classifications; HUBZone, 8(a), Small Disadvantage Business, Woman Owned Business, Veteran Owned Business, Service Disabled Veteran Owned Business relative to NAICS Code 621111, with a small business size standard not to exceed $10,000,000.00 gross annual receipts. Responses to this sources sought synopsis will be used by the Government to make appropriate determinations about potential sources only. No solicitation is currently available at this time. After review of the responses to this sources sought announcement, a synopsis and/or solicitation announcement may be published at a later time. Responses to this notice shall include company name, address, point of contact, contact information to include phone and e-mail address, size of business pursuant to North American Industrial Classification Code (NAICS) and must respond to the following questions: 1)Is your business a small business under NAICS code 621111? 2)Does your business provider Neurosurgery Physician services? 2) Does your firm qualify as a small disadvantaged business? 3) If disadvantaged specify if your firm is certified under Section 8(a) of the Small Business Act? 4) Are you a certified HUBZone firm? 5) Are you a woman-owned or operated business? 6) Are you a certified Service-Disabled Veteran Owned or Veteran Owned business? 7) Is your business a large business? Please provide a capability statement addressing your organization's ability to perform as a prime contractor the work described in the draft PWS (below). Index your firm's capabilities to the tasks in the draft PWS so that your firm's capabilities can be readily assessed. This is not request for proposal and in no way obligates the Government to award any contract. Please provide responses in writing no later than June 15, 2012 to: Jenny.Rathjen@va.gov Telephone responses will not be accepted. Responses to this sources sought announcement will not be considered as a response to any solicitation, a request to be added to a prospective offerors list, or to receive a copy of any solicitation. No solicitation is currently available at this time. DRAFT Performance Work Statement 1.SERVICES 1.Contractor shall provide the services of 1.0 FTE neurosurgeon physician to perform neurosurgical case related services which will include: a.Operation of neurosurgery clinics at the John Cochran Division. (1)One morning clinic of four (4) hours length, per month. Clinic will be held on designated (regularly scheduled) day which will be mutually agreed upon by both the contractor and VA Medical Center (2)One morning clinic of four (4) hours length, per week. Clinic will be held every Friday morning (4 hours) (3)One full-day clinic for eight (8) hours per week. Clinic will be held on designated (regularly scheduled) day which will be mutually agreed upon by both the contractor and VA Medical Center. (4)Clinics days and times may be changed when mutually agreed upon by the contractor and the VA Medical Center. b.To appropriately respond to half (50% ) of the neurosurgical consultations placed at the VA Medical Center twenty four hours per day, seven days a week, 365 days a year. It is understood that during telephone consultations the contractor's employees may not be on site at the VA facility. c.Provide OR, outpatient clinic and in patient coverage for the on-site VAMC Neurosurgeon when the on-site VAMC Neurosurgeon is unavailable due to leave, illness, or authorized absence. d.Provide neurosurgery procedures at the John Cochran Division including but not limited to the following: Laminectomy, Ruptured Cervical Disc, Hemilaminectomy and Discectomy, Diskectomy, Cranioplasty, Diskectomy/Laminectomy, Craniotomy, Lobectomy, Subdural Hematoma, Craniotomy/Subdural Hematoma and other neurosurgery procedures as determined appropriate by Program Manager, Surgery, and the contractor. e.There will be a designated weekly OR time, mutually agreed upon, for scheduling elective surgeries. VA Medical Center will accommodate additional OR time(s) in order to meet emergency, urgent, high volume patient care needs. Additional OR time (s) will be mutually agreed upon. 2.The neurosurgeon's work consists not only of providing intraoperative neurosurgery expertise at the St. Louis VAMC but also includes a clear cut personal involvement as evidenced in the medical record by a physical presence at the bedside, in the clinic, and, when necessary, phone conversations where VA patients are discussed. 3.Duties and Responsibilities for Neurosurgeon a.Clinical availability and time slots are locked into the Veterans Integrated Systems and Technology Architecture (VISTA) System. Requests to change the clinical profile, cancel clinic, or prohibit scheduling for the clinic must be submitted to and concurred with by the VA COR. b.The contractor shall ensure all operative reports are dictated within 24 hours of surgery and signed within 10 working days after the report has been dictated. All discharge summaries are to be dictated prior to discharge but no later than seven days post discharge and signed within 10 calendar days of patient discharge. All patient workload capture processes (Encounter Forms) and all progress notes/consultations are typed into the CPRS and electronically signed off on the same day of the clinic visit. 4.Response Time. a.For acute surgical emergencies, the contract physician must respond within 30 minutes of notification. Acute surgical emergencies will be determined by the contract physician via telephone consults with VA staff. For routine cases, the contract physician must be present within twenty four (24) hours or the next business day. b.Outpatient Services. All attempts shall be made to see patients by the contract physician within the current standards for waiting time. Patient appointments shall be scheduled so that new patients are given half hour appointments and follow up patients are given 15 minute appointments. The John Cochran facility is to provide all patient files, tests and x rays on clinic day at the Clinic for the contract physicians' review. Patients referred to the outpatient clinic will receive an appointment with the contract physician for evaluation within 30 days after referral. If the situation is deemed urgent, an alternative earlier examination or opportunity for evaluation will be sought. 5.Timeliness of Consults. Contractor shall be required to meet with the patient and provide a written recommendation within twenty four hours, or the next business day, upon receiving a consult request. 6.Coverage When Contractor Is Not Physically Present. When a change in a patient's neurosurgical condition occurs of critical nature during off duty hours, the on call contract Neurosurgeon must immediately be contacted to evaluate the patient. Documentation of the contract physician's discussion of these events shall be made in the electronic medical record within twenty four (24) hours or within the next business day by the contractor and be co-signed by the VAMC Neurosurgeon. The contractor must respond or must ensure that another qualified and privileged neurosurgeon is available to respond within 30 minutes if it is determined by the contract physician that the condition is of a critical nature and a physical presence is necessary during off duty hours. Arrangements should be made that the patient shall be examined by a Neurosurgeon the following work day. If the change in a patient's condition is of a non critical nature or there is an incident of a non critical nature during off duty hours, the contract physician shall make arrangements for the patient to be scheduled into a clinic appointment slot, as deemed appropriate. 7.Quality Assurance Responsibilities. a. The contract physician's staff are expected to actively participate in the Medical Center's performance improvement activities involving the Surgical Program. At the request of the VAMC Chief of Surgery, Patient Safety Manager, Quality Management Director, and/or Risk Manager, the contractor shall be expected to provide information to assist the Surgical Program in monitoring and evaluating their performance within a specified time frame. These time frames shall vary, depending on the nature and urgency of the specific issue/situation. Conclusions, recommendations and follow up actions taken shall be made to ensure ongoing improvement in patient care services. a.Performance Improvement Activities shall include the following: (1)Surgical Care Review (2)Surgical Risk Assessment Program (3)External Peer/Case Review Program Reporting (4)Infection Control (5)Blood Usage Review (6)Disaster Plans (7)Hospital Safety (8)Outpatient Qualitative and Quantitative Review (9)Medical Record Review (10)Drug Usage Evaluation (11)Internal Peer Review (12)Patient Incident (13)Utilization (14)Adhere to Surgical Service or Hospital initiatives to improve patient safety, operating room efficiency, and/or patient flow. a.The contracting institution shall share relevant QI data (e.g. minutes from Morbidity and Mortality Conference) for patients who receive neurosurgical care at the contractor's institution. b.All patient clinic Encounter Forms and all progress notes/consultations are typed into the CPRS and electronically signed off on the same day of the clinic visit. c.Requests for consultation shall be made from any services in the medical center via the computerized patient record system (CPRS). Non emergent requests for consultation are usually made by scheduling patients for clinic appointments. Emergency consultations are usually telephonic, and more often than not, between physicians. 2.LOCATION OF SERVICES Physician services will take place at the Saint Louis VAMC-John Cochran Division.
