Description

SOURCES SOUGHT ANNOUNCEMENT: The Lebanon VA Medical Center has a requirement for the provision of medications and appropriate infusion supplies and equipment, provision of skilled nursing services to educate family/caregivers in the use of medication and supplies, reason for the provision of the medication and any possible side effects to be aware of, administration and monitoring of medications given per Physician orders coupled with this is the need for Home IV Infusion required to perform all pharmacy compounding using aseptic techniques in accordance with USP and provide all other in home skilled care and supervision necessary to provide home infusion therapy services in conjunction with home infusion if ordered by physician to eligible veterans of the Lebanon VA Medical Center (LVAMC). Estimated period of performance is base year plus 4 option years. Estimated date of award is: October 1, 2012. This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. This notice is to obtain possible sources that have the capacity and capabilities to include necessary equipment, personnel, licenses, insurance and qualifications to furnish home IV infusion and Skilled Nursing. The North American Industry Classification System Code (NAICS) is 621610 and the size standard of $13.5 Million will apply to this solicitation. The solicitation package will be available approximately on or about June 29, 2012. Responses to this notice should include the following: 1.Offeror's company name, DUNS, address, point of contact, title, phone number and email address. 2.Offeror's capability to meet this requirement and any pertinent information which demonstrates the firm's ability to meet the above requirements. 3.Offeror's must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals, must perform all quality control procedures as required by Federal and State regulatory agencies, and must meet all Nuclear Regulatory Commission, Department of Transportation, FDA, OSHA and all other agency rules and regulations (Federal and State). 4.Offeror's type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Women Owned, Small Disadvantaged, etc. If applicable, Service-Disabled Veteran Owned Small Business (SDVOSB) must be verified in VetBiz. All responses shall be submitted in writing no later than 1200 (noon) June 11, 2012 EST to ATTN: Crystal Roberts (125), VAMC Lebanon, 1700 South Lincoln Ave. Lebanon, PA 17042. E-mail: crystal.roberts4@va.gov. Facsimile or telephonic responses will not be accepted. Simply responding as an "interested party" in www.fbo.gov doesn't constitute your company as a source. Offeror must provide the above requested information. After review of the responses to this sources sought notice, a solicitation announcement may be published on the FBO website in the near future. Responses to this sources sought announcement are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought notice. All interested parties are reminded to be registered with Central Contractor Registration (CCR) at http://www.ccr.gov in order to be eligible for award of Government contracts.
