Description

The Network Contracting Office (NCO) 12, Great Lakes Acquisition Center, is soliciting offers to provide all personnel, material, equipment, and supervision necessary to perform Dermatologist Services for the William S. Middleton Memorial Veterans Hospital, Madison, Wisconsin. The contract period of performance will be from the date of award (anticipated August 1, 2012) through July 31, 2013, with four (4) one year option periods. Option periods are exercised at the sole discretion of the Government. The contract type is firm-fixed unit price indefinite-delivery, indefinite-quantity (IDIQ) contract. This procurement is set-aside 100% for small businesses. All qualified offeror/firms who believe they can provide the required services are encouraged to submit an offer. The small business size standard for North American Industry Classification System (NAICS) code 621111 is $10.0 Million. Offerors are reminded to complete all sections of the solicitation including the FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. The electronic version of solicitation will be available on or about June 8, 2012. Date for receipt of offers is July 6, 2012, 10:00 A.M. CST. The electronic solicitation may be obtained FREE at the Federal Business Opportunities website: www.fedbizopps.gov. Any amendments to this solicitation will also be available at this address. Offerors are advised that they are responsible for obtaining any amendments which are to be submitted with your response.
