- Request for Quotation and Scope of Work for Solicitation 12-161-SOL-00024 Note; This acquisition is a total small business set-aside. The applicable North American Industry Classification System (NAICS) code is 238210 -- Electrical Contractors and Other Wiring Installation Contractors. The Size Standard used to determine a business's size is $14 Million average annual revenue. Only firms meeting this size standard will be considered under this Notice. Description of the Work. The Contractor shall conduct a complete Arc Flash Hazard Analysis in compliance with IEEE 1584 and 2009 NFPA 70 E. Arc flash hazard analysis is defined as a study investigating a worker's potential exposure to arc-flash energy conducted for the purpose of injury prevention and the determination of safe work practices, arc flash protection boundary, and the appropriate levels of Personal Protective Equipment (PPE). (2009 NFPA 70E, article 100). Various services, detailed below, shall be performed at the following IHS facilities: Duck Valley Health Center Nevada Hwy 225 Owyhee, Nevada 89832 Whiteriver Health Center 102 West Hospital Drive Whiteriver, AZ 85941 Desert Visions Youth Treatment Center 198 Skill Center Road Sacaton, AZ 85247 Nevada Skies Youth Treatment Center 104 Big Bend Ranch Road Wadsworth, Nevada 89442 Hopi Health Care Center HWY 264, MM 388 Polacca AZ 86042 Parker Indian Health Center 12033 Agency Rd Parker AZ 85344 Ft Yuma Clinic Indian Hill Rd Winterhaven CA Uintah & Ouray Clinic Ft Duchesne UT 84078 San Carlos Indian Hospital San Carlos AZ 85550 Bylas Clinic and Dental Clinic Bylas AZ Peach Springs Health Center Peach Spring AZ 86434 Walker River Health Center Schurz NV a. Arc Flash Hazard Analysis and Training. The contractor shall perform the following tasks at Duck Valley, Walker River, Desert Visions, Nevada Skies and Bylas. i.Provide labor and materials to collect the necessary information from the site electrical including wire lengths, types and sizes, conduit types and sizes, over current device including all circuit breakers as well as all disconnects. ii.Verify the existing one line accuracy and all major components within the electrical system. iii.Develop a short circuit study. iv.Develop a coordination study and discuss any issues with poor or inappropriate coordination of over current devices. v.Develop an Arc Flash Hazard Analysis. vi.Produce and install permanent labeling on all equipment identified for arc flash notification. Note that each label will be unique to the equipment (PPE) required, the potential calories per square centimeter at 18-inch and exposure levels should an arc flash occur as well as distances in inches for the various boundaries required by NFPA 70E. vii.Training shall be in accordance with 2009 NFPA 70E, article 110 and shall include but not limited to: a)Classroom and on-the-job instruction shall be provided to between 5 and 20 people on: •Requirement for the Arc Flash Analysis •the findings of report, •explanation of the one-line diagrams •explanation of the label system •demonstration of the appropriate PPE b)The contractor/trainer shall develop training rosters for each service unit consisting of staff members requiring training. This list will be presented to each service unit Facility Manager for review and consideration. c)Scheduling of the training shall be at the discretion of the Facility Manager to ensure that the appropriate individuals attend. d)The Contractor shall conduct a tour of the facility as part of the training event to review each piece of equipment or panel that was labeled and the corresponding aspect of the one-line diagram. e)The Contractor shall provide documentation of the training in accordance with 2009 NFPA 70E, article 110.6(E). The documentation shall be completed when the employee demonstrates proficiency in the Arc Flash Analysis and appropriate aspects of the training. b. Site Electrical Information. The Contractor shall perform the following task at Whiteriver, Hopi, Parker, Yuma, U&O, San Carlos, and Peach Springs: i.Provide labor and materials to collect the necessary information from the site electrical including wire lengths, types and sizes, conduit types and sizes, over current device including all circuit breakers 200 amps and smaller as well as all disconnects of 100 amps and smaller at all listed sites. c. Report Preparation. Provide a complete report to include a new one line, one line with arc flash values for the devices, coordination study if necessary and short circuit report with amps interrupting capacity violations. d. Ground and Bonding Study. The contractor shall perform a grounding and bonding study of the Bylas Dental Clinic. The study shall ascertain whether the recently constructed building is in compliance with 2011 National Electric Code, Article 250. The contractor shall provide the findings in report format along with proposed corrections to deficiencies to bring the installation into compliance with article 250 of NEC.
