- Donkey Creek Aquatic Organism Passage & Road Treatment. Project work consists of the following major items of work: replacing a culvert with a low profile open bottom arch; construction of a single lane bypass at the crossing (suitable bridge or approved fill bypass); traffic control signing and barriers meeting MUTCD; construction survey and staking of a road and stream channel; coordinate work schedule with Grays Harbor PUD due to buried powerline relocation; contractor testing; soil erosion & pollution control; clearing & grubbing; dewatering the site during construction; removal of existing culverts ( two 6-foot diameter pipes); structural excavation and backfill; installing reinforced concrete footings; installing structural plate open bottom arch (approximately 32 foot span X 11 foot rise X 50 foot length); placing a stream simulation rock streambed; placing riprap; placing, shaping and compacting aggregate subgrade and asphalt pavement. This project is located on the Pacific Ranger District of the Olympic National Forest: Sections 10, Township 21 N, Range 09 W, W.M., near Quinault, Washington. It may be reached by traveling south from Quinault, Washington, on HWY 101 for approximately 12 miles to Forest Service Road (FSR) No. 22 (Donkey Creek Road). Proceed on FSR 22 for approximately 8.2 miles to the project site at the tributary of Donkey Creek. Project site is two steel culverts under a two lane paved roadway. Period for performance is from September 30, 2012, to October 15, 2013. Project Manager is Mr. Shannon Henriquez, 360-956-2264. The Forest Service will proceed to issue a solicitation as a Request for Proposals (RFP) on or about July 12, 2012. This acquisition is 100 percent set-aside for small business concerns. The Forest Service will award a firm fixed-price contract to that responsible Offeror whose proposal, conforming to the solicitation, is fair and reasonable, and has been determined to be the best value to the Forest Service. Offers will be evaluated based upon: price; experience, past performance, managers & subcontractors, price realism, and a Level of Confidence Assessment Rating (LOCAR). The government estimated value is between $250,000 and $500,000. A bid bond is required. Performance and payment bonds are required for this project. Davis-Bacon wage rates apply to any resulting contract. The proposed contract-type is firm-fixed price. All responsible sources may submit an offer which will be considered by the government. In accordance with FAR 4.1102(a), prospective contractors must be registered in the Central Contractor Registration (CCR) database prior to award of a contract. Lack of registration in the CCR on the part of the offeror will render an offeror ineligible for award.
