Description
Infectious Waste Disposal Service This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Parts 12 and 13, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/). The RFQ number is N00604-12-T-3153. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-58 and DFARS Change Notice 20120629. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acqnet.gov/far and http://www.acq.osd.mil/dpap/dfars/index.htm. The NAICS code is 562219 and the Small Business Standard is $12.5M. The proposed contract is 100% set aside for small business concerns. The NAVSUP FLC Pearl Harbor requests responses from qualified sources capable of providing: Infectious Waste Disposal Services for two Naval Health Clinic Facilities on the Island of Oahu, Hawaii IAW the attached Performance Work Statement. CLIN 0001 - Makalapa Clinic Base Year (01-August-2012 through 30-July-2013), 104 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. CLIN 0002 “ Kaneohe Bay Clinic Base Year (01-August-2012 through 30-July-2013), 60 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. CLIN 1001 - Makalapa Clinic Option Year 1 (01-August-2013 through 30-July-2014), 104 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. CLIN 1002 “ Kaneohe Bay Clinic Option Year 1 (01-August-2013 through 30-July-2014), 60 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. CLIN 2001 - Makalapa Clinic Option Year 2 (01-August-2014 through 30-July-2015), 104 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. CLIN 2002 “ Kaneohe Bay Clinic Option Year 2 (01-August-2014 through 30-July-2015), 60 each, weekly transportation, treatment and disposal of infectious waste services, 95 gallon bin. Period of performance is base year 01-August-2012 to 30-July-2013, option year 1 01-August-2013 to 30-July-2014, and option year 2 01-August-2014 to 30-July-2015; Service Location is Makalapa Clinic Joint Base Pearl Harbor Hickam, Hawaii 96860 and Kaneohe Bay Clinic Kaneohe Bay, Hawaii 96863. Responsibility and Inspection: unless otherwise specified in the order, the supplier is responsible for the performance of all inspection requirements and quality control.
