- This is a combined synopsis/solicitation for repair service prepared in accordance with the format in Subpart 12.6 as supplement with additional information included in this notice. This announcement constitutes the only solicitation to be sent out; proposals are being requested and a written solicitation will not be issued: Solicitation #CCHCF-12-54001: This combined synopsis/solicitation is issued as a Request for Quote (RFQ). NAIHS CODE: 237110. Description: The requirement is for a firm-fixed price (FFP) type purchase Order. The Contractor shall provide on-site Tennis Court Repair Services as scheduled by the Chinle Maintenance Department. Delivery and work shall be performed Monday through Friday between 8:00am to 5:00pm (Mountain Standard Time). The Contractor shall provide all management, supervision, labor, materials, supplies, equipment, and transportation. Contractor shall perform the Following: 1. Contractor will re-level the 120' x 120' court area and compact the site. 2. Contractor will compact court site with 2" ABC gravel (107 cubic yards and/or 187 tons) according to specifications. 3. Contractor will erect forms; install wire mesh, expansion joints, under netting and rebars according to specifications. 4. General clean up and preparation. 5. Four pours of 155 cubic yards of concrete at 4,000 psi, with air and mixed with fiberglass. Concrete will withstand potential cracking & breaking with proper fiberglass addition. 6. Concrete lay down with proper gravel spad down, erect pipes for tennis nets, float evenly and set ready for finishing. 7. Power trowel concrete slab as required for a professional finish approach. 8. Hand trowel finish the concrete to specifications. 9. General clean up and trash haul. Proposal only set aside for Service Disable Veteran Owned Small Businesses, The Interested firms are invited to respond by sending their proposals with the following: (1) Announcement number, (2) Company name, DUNS, EIN, address, telephone and fax number and (3) cost breakdown of proposal. IHS will not pay for any cost incurred associated with this Request for Proposal. As part of the evaluation process, quotations submitted by other than Service Disable Veteran Owned Small Business will not be considered this is a special set aside for Service Disable Veteran Owned Small Businesses only.
