- Solicitation Clauses and Provisions Technical Exhibit #4, Pre-Diabetes Guidelines Technical Exhibit #3, Health Program List Technical Exhibit #2, Reports Technical Exhibit #1, Performance Requirements Summary (PRS) Quality Assurance Surveillance Plan (QASP) Performance Work Statement (PWS) (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 12-235-SOL-00021 is issued as a request for proposal (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. (iv) This acquisition is a total small business set aside. The NAICS code is 541990 and the size standard is $14,000,000. (v) There is only one contract line item which is to provide diabetes case manager and GPRA consultant services in accordance to the Performance Work Statement. (vi) The contractor shall provide all services required in the Performance Work Statement (PWS). (Attached) The contractor's performance of the contract will be evaluated by the criteria set forth in the Quality Assurance Surveillance Plan (QASP). (Attached) (vii) The period of performance is one year from time of award. (viii) The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition (attached). (ix) The provision 52.212-2, Evaluation-Commercial Items, applies to this acquisition (attached). (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer (attached). (xi) The clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition (attached). (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition (attached). (xiii) All contract clauses and provisions are attached to this synopsis/solicitation. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition.
