Description
THIS IS A REQUEST FOR SF330 FORMS ONLY. THERE ARE NO SOLICITATION DOCUMENTS AVAILABLE. The Department of Veterans Affairs, VA Medical Center West Palm Beach FL is soliciting and intends to award a firm fixed price contract for Architect/Engineering (A/E) services for development of complete construction documents (i.e. working drawings, specifications, and reports), Certified Independent Third Party Safety Professional services, and construction period services for Project 548-12-107, entitled Create Admin Support Center. Contractor shall design a project to create a minimum of seven (7) office spaces with waiting areas in the main atrium (1B-200) located at the Medical Center, 7305 N Military Trail, West Palm Beach, FL. Each office will comply with VA design standards for Ambulatory Care (Hospital Based) Office. Total space to be renovated is approximately 2500 square feet. Interested firms shall have the capability of engaging all additional disciplines as necessary to provide a complete proposal. Estimated cost range for construction is between $500,000 and $1,000,000. NAICS code for this project is 541310 and the Small Business Standard is $7.0M. Set-Aside/Area of Consideration: This acquisition is set-aside for Service Disabled Veteran Owned Businesses (SDVOBs) who are currently registered in the Central Contractor Registration (CCR) database http://www. ccr.gov, US Department of Labor Vets100 database: https://vets100.vets.dol.gov, Vet Biz database: http://www.vet biz.gov, and On Line Representations and Certifications (ORCA): https://orca.bpn.gov. Interested parties must be registered in these databases and registration must be current, otherwise the proposal will be rejected as ineligible to participate. The contract will be awarded to a firm whose registration in the aforementioned databases is current. NOTE: THIS SHALL BE A SERVICE-DISABLED, VETERAN-OWNED, SMALL BUSINESS SET-ASIDE. PROSPECTIVE CONTRACTORS ARE CAUTIONED THAT ANY BID OR PROPOSAL SUBMITTED IN RESPONSE TO THIS SOLICITATION MUST MEET THE CRITERIA IDENTIFIED BY 38 CFR, PART 74. ANY PERSON, PERSONS, OR BUSINESS ENTITY SUSPECTED OF MISREPRESENTING ITSELF FOR THE PURPOSE OF SECURING A GOVERNMENT CONTRACT MAY BE CRIMINALY INVESTIGATED AND PROSECUTED FOR FRAUD AGAINST UNITED STATES OF AMERICA. PARTIES FOUND MISREPRESENTING THEIR STATUS ALSO RISK DEBARMENT FROM FURTHER GOVERNMENT CONTRACTS. To be considered for this award, contractors need to submit a completed Standard Form 330 (One hard copy and one CD copy). SF 330 should be submitted no later than noon, on Friday, August 10, 2012 to VA Medical Center WPB, Bldg 12, Attn: Jose Delgado, 7305 N Military Trail, West Palm Beach FL 33410. The SF330 may be downloaded at www.gsa.gov/forms. Government-wide Number Note 24 is applicable. Questions concerning this solicitation should be e-mailed to the Contracting Officer at Jose.Delgado@va.gov no later than noon, August 1, 2012. Questions received after the deadline will not be answered. Telephone inquiries will not be accepted. In accordance with FAR 36.602-1 and VAAR 836-602-1, firms will be evaluated based on the following evaluation criteria which are shown in descending order of importance: (1) Professional qualifications necessary for satisfactory performance of required services; (2) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials; (3) Capacity to accomplish the work in the required time; (4) Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules; (5) Location in the general geographical area of the project and knowledge of the locality of the project; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project. (6) Reputation and standing of the firm and its principal officials with respect to professional performance, general management, and cooperativeness. (7) Record of significant claims against the firm because of improper or incomplete architectural and engineering services. (8) Specific experience and qualifications of personnel proposed for assignment to the project and their record of working together as a team. Firms will be evaluated by a selected VA board whose evaluations will include, but are not limited to the evaluation criteria listed above. Firms will be scored and ranked according to a score of Poor, Marginal, Acceptable, Good, and Outstanding. The three (3) most qualified firms will be invited to interview with the selection board and will again be adjectively scored. The firm with the highest score will be selected and asked to submit a detailed cost proposal and negotiate a fair and reasonable price.
