- SOURCES SOUGHT NOTICE This is NOT a solicitation for proposals or quotes. The purpose of this notice is to obtain information regarding: General Statement of Need: The availability and capability of qualified business sources that can provide the following Human Free Light Kappa and Lambda Reagent kits for use on Siemens BNII analyzer for VISN 1 Pathology and Laboratory Medicine Service (P&LMS) for testing in the Laboratory at the VA Medical Center in West Roxbury, Massachusetts, as requested by authorized VA Staff. All supplies provided under this contractor shall be F.O.B. Destination; fuel surcharges shall not be allowed nor added to the invoice. The VABHS is seeking one responsible vendor that can provide all of the supplies listed below. Supplies requested are as follows: Product Product # Yearly Qty HUMAN FREE KAPPA KITS # LK016.t30 HUMAN FREE LAMBDA KITS#LK018.t30 Note: Estimated quantities are based upon a 12-month period. 1.DELIVERY ORDERS: The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA order number. Orders can be placed orally by telephone and/or fax transmission on an "as needed" basis. 2.DELIVERY LOCATION: VA Medical Center, 1400 VFW Parkway, West Roxbury, MA The NAICS code(s) for this service is 334516. The contract duration will be a base year with four option years. Offerors having the capabilities necessary to provide the stated supplies are invited to provide information to contribute to this Sources Sought notice including commercial market information and company information via e-mail to Yolanda Cotten at Yolanda.Cotten@va.gov, no later than July 17, 2012. RESPONSES SHOULD INCLUDE THE FOLLOWING INFORMATION: company name, address, point of contact, phone number, and e-mail address.
