Description
The Department of Veteran Affairs, NCO 7, Atlanta VA Medical Center is seeking responsible sources to provide service as outlined in the Statement of Work. This is market research for informational and planning purposes only. This is not a solicitation and does not obligate the government to issue a solicitation. This is to gain knowledge of potential qualified sources related to NAICS 541711. Please be advised that the Government will not pay for any information submitted under this announcement. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement and identify your business size, and if you are any of the following: Federal Supply Schedule holders, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub-Zone Small Business, Small Disadvantage Business, and Women Owned Small Business. Responses to this notice shall be e-mailed to Jessica Coleman-Smith, Contract Specialist, E-mail address: jessica.coleman-smith@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 3:00PM EST on July 25, 2012. Responses should clearly state the ability to provide the service. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately. STATEMENT OF WORK A. GENERAL GUIDANCE 1. Title of Project: Development of Smart Phone Apps for Visually Impaired 2. Scope of Work: Contractor shall provide Biomedical Design Services to develop software and hardware in support of RR&D Projects on Low Vision or visually impaired veterans. The contractor will interact and consult with the PIs for the development of various applications for visually impaired individuals. 3. Location of Services Provided: Atlanta VA Medical Center, 1670 Clairmont Rd, Decatur, GA 30033, RR&D 12th Floor. 4. Work Hours: The contractor shall be a available during normal business hours of 8:00am - 2:30pm Monday - Friday excluding Federal Holidays. 5. Federal Holidays: New years Day, Martin Luther King's Birthday, President's Day, Independence Day, labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. 6. Type of Contract: This shall be a firm fixed hourly rate priced contract. B. GENERAL REQUIREMENTS: 1. An access badge will be given to the contractor upon entrance into VA buildings. The contractor must safeguard the access badge and immediately report any lost, stolen, or destroyed badges to the COR. All contract personnel must properly display their access badges. Access badges must be worn at or above the waist (facing forward.). The contractor must return the access badge(s) to the COR or designee at the end of this contract. 2. It is the responsibility of the contractor's personnel to park in the appropriate designated parking areas. Parking information shall be coordinated with each facility COR. 3. Atlanta VAMC does not validate or make reimbursement for parking violations of the contractor's personnel under any circumstance. C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES 1.The Contractor shall: Provide biomedical design support to the VA Rehabilitation Research and Development Merit project awarded to Dr. James Marston. Contractor will provide analysis of data, collection of data and development of software applications for the smart phone for visually impaired veterans. Contractor will be responsible for helping organize administrative paperwork (e.g. maintaining the regulatory binder & managing IRB/R&D paperwork). He will provide regular internal consistency reports in order to minimize testing-related errors that could affect data integrity. 2.Description of Service: For the purpose of this contract, the following are descriptions of the types of services to be performed. "Training of other PIs "Software Design development "Availability for trouble shooting via telephone or e-mail D. REPORTING REQUIREMENTS: "The Contractor shall report to the RR&D Center on the scheduled days. " The Contractor shall submit the following documentation to the Contracting Officer's Technical Representative at the end of each week. a.Invoice of hours worked b.Description of work performed E. QUALITY ASSURANCE: PERFORMANCE STANDARDS REQUIRED SERVICEPERFORMANCE STANDARDMONITORING METHODINCENTIVE/DISINCENTIVES FOR MEETING OR NOT MEETING THE PERFORMANCE STANDARDS Software design of smart phone apps for visually impairedContractor will meet weekly with PI for update on Project progressSupervision of PI as to progress of application development5% of the total invoice amount shall be deducted when the performance standard is missed F. GOVERNMENT RESPONSIBILITIES: The Government shall be responsible for making system available to Contractor during normal business hours 8:00 am - 4:30 pm. If there is a need for work to be done after hours, the Government will make arrangements for Contractor to have access to system. G. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS The contractor shall be required to comply with all security policies/requirements. All security policies/requirements must be met and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract. H. INTERFERENCE TO NORMAL FUNCTION: N/A
