Attachment 1 - Price Quotation Matrix-_RFQ_HSTS04-12-Q-CT7023 HSTS04-12-Q-CT7023 EDS Bearing Analysis RFQ The solicitation number is HSTS04-12-Q-CT7023. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This Request for Quotation (RFQ) is issued for EDS Bearing Analysis. Offerors are requested to review the attached EDS Bearing Analysis RFQ and submit their quote in 2 parts: Volume 1 and Volume 2 Price. In addition, offerors are requested to complete the attachment Price Quotation Matrix. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-58. This acquisition is 100% set-aside for small business concerns. The North American Industry Classification System (NAICS) number is 332991. Small Business size standard is 750 employees. List of contract line item number(s): See attached EDS Bearing Analysis RFQ. Description of requirements for the items to be acquired: See attached EDS Bearing Analysis RFQ. Date of Delivery: See attached EDS Bearing Analysis RFQ. Place of delivery and performance: See attached EDS Bearing Analysis RFQ. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, however no alternative terms and conditions or commercial items outside those specified in the attached EDS Bearing Analysis RFQ will be considered. The provision at 52.212-2, Evaluation -- Commercial Items, does not apply. The evaluation process used is Lowest Price Technically Acceptable. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer.
