- STATEMENT OF WORK revised COMBINED SYNOPSIS/SOLICITATION INDUSTRIAL HYGENTIST SUPPORT FOR CONFINED SPACE PROGRAM (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number WE133M12RQ0792 (III) The solicitation document and incorporated provisions and clauses are those in effect through: Federal Acquisition Circular 2005-58. (IV) This solicitation is being issued as a Total Small Business Set-Aside The associated NAICS code is 541330. The small business size standard is $4.5 M (V) This combined solicitation/synopsis is for purchase of the following commercial: REQUESTING AN INDUSTRIAL HYGENTIST FOR SUPPORT ON NOAA’S CONFINED SPACE PROGRAM IN NEWPORT, OR. (VI) Description of requirements is as follows: See attached STATEMENT OF WORK (VII) Date(s) and place(s) of delivery and acceptance MARINE OPERATIONS CENTER – PACIFIC 2002 SE MARINE SCIENCE DRIVE NEWPORT, OR 97365 Delivery shall be FOB Destination NEWPORT, OR. Period of performance shall be: July 31 ST, 2012 through August 1 st, 2013. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (March 2012) applies to this acquisition. In addition to written price quotes, offers are instructed to provide (IX) (IX) FAR 52.212-2, Evaluation – Commercial Items ( March 2012), applies to this acquisition. Paragraph (a) is hereby completed as follows: Offers will be evaluated by trade-off with the majority of the weight contributed to technical expertise and past performance.
