- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 13 and with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number is 1107467, and is issued as a Request for Quotation. The solicitation documentation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58, Dated May 18, 2012. The associated North American Industry Classification System Code is 811219 - Other Electronic and Precision Equipment Repair and Maintenance with a small business size standard of $19.0 million. This requirement is set aside for small businesses. The Government requires a maintenance agreement for CEM MARS Explorer and Discover Systems for a base period of one year 09/06/2012 through 09/05/2013 with two one-year option periods (to be separately priced). The equipment is installed at 3900 NCTR Road, Jefferson, AR. Two annual system PM inspections The provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this acquisition. The offeror shall complete only paragraph (b) of this provision if the offeror has completed the Annual Representations and Certifications electronically at http://orca. bpn.gov. In addition, the Contractor shall provide monthly reports to the FDA COR and Contract Specialist, not later than the 5th work day following the end of each month, summarizing all maintenance and repair activities (including warranty work) for the previous month; negative reports are required. The following factors shall be used to evaluate offers: (1) Technical Capability: Parties (other than Original Equipment Manufacturer) responding to this RFQ must provide certification from the Original Equipment Manufacturer (OEM) that they are qualified to perform maintenance and repairs as well as have access to parts, service manuals and all other tools required to perform this maintenance requirement to include all firmware and software upgrade and updates. (2) Past Performance: Offerors (other than OEM) must furnish information for a minimum of three maintenance contracts for the above equipment including customer contact point and telephone number. (3) Price (inclusive of option periods) Technical capability and past performance, when combined, are more important than price. Offerors are afforded the opportunity to inspect the system by contacting the Contract Specialist/Contracting Officer identified herein to schedule an appointment. Failure to inspect the system will not relieve the successful offeror from fully meeting the requirements of the resulting contract at the price offered. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition with the following addenda: Offerors are afforded the opportunity to inspect the system by contacting the Contract Specialist/Contracting Officer identified herein to schedule an appointment. Failure to inspect the system will not relieve the successful offeror from fully meeting the requirements of the resulting contract at the price offered. Offerors who fail to complete the required representations and certifications, or reject the terms and conditions of this solicitation may be excluded from consideration.
