12-233-SOL-00569 - Notice of Intent THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A QUOTE. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND QUOTES WILL NOT BE REQUESTED. Based on the attached statement of work the Program Support Center (PSC) on behalf of the U.S. Department of Health and Human Services, Office of the Assistant Secretary for Health (OASH) intends to award a fixed priced sole source purchase order in accordance with 13.106-1(b), to the Public Health Foundation Enterprises, Inc., 12801 Crossroads Parkway South, Suite 200, City of Industry, CA 91746. This award will have a 12 month period of performance from September 29, 2012 - September 28, 2013. This acquisition is being conducted under FAR Part 13, Simplified Acquisition Procedures, and is exempt from the requirements of FAR Part 6, Competition Requirements. The acquisition shall not exceed the simplified acquisition threshold. The North American Industry Classification (NAICS) Code is 541690 and the Size Standard is $14.0 M. The Public Health Foundation Enterprise (PHFE) is currently managing the staff for this project on behalf of the California Department of Public Health (CDPH). PHFE is a bona fide agent for the state of California's Epidemiology and Lab Capacity (ELC) grant with CDC, a project closely related to the Healthcare-Associated Infection (HAI) project. In the current contract the contractor conducted site visits where they have met with and obtained input from many different groups around the state involved in Health-care associated Infections (such as county health officials, hospital infection-control staff, quality improvement specialists, hospital nurses, physician-based office staff, etc). Technical assistance has been provided on most of these visits, with significant 1:1 follow-up occurring by telephone. These outreach efforts have resulted in the formation of professional relationships that are the backbone of these projects and are an integral part of being able to assure successful outcomes. By providing this technical assistance, teaching and showing how to implement industry standards, and assisting with the data retrieval and report generation parts of the project, trust has developed and the ability for honest exchange that has allowed for true learning and improvement to occur. These relationships are critical to the success of the project. When sites make mistakes or discover large problems that need to be corrected, they now have a trusted resource to whom they can turn for advice and guidance with confidence. The key outcomes of this project - reduce CLABSIs by 50% ; achieve 100% compliance with CLIP bundle; reduce hospital onset of CDI by 30%; reduce incidence of MRSA BSI by 25%; and reduce deep and organ space SSI by 25% - as well as the continuation of oversight of the HAI prevention collaboratives, involving groups of hospitals and healthcare providers who are actively working on C difficile infection prevention and improved antimicrobial stewardship, HAI prevention best practices in very smallest hospitals (<30 beds), and "On the CUSP: Stop CAUTI," are all based on the maintenance of these trusting relationships that have developed over time. Participants must have an infection preventionist who they trust when it is time to input new data, review results, discuss implementation of standards, and to have questions answered in a non-threatening way. Another deliverable in this project is development of a communications plan. In order to develop this, the above site visits and relationships have been cultivated in order to obtain meaningful, ongoing input from the sites as they work toward the achievement of the project goals and outcomes. While information has been delivered and reports have been generated on these site visits, the "institutional knowledge" gained that doesn't lend itself to being captured in reports will be crucial to the crafting of a useful, authentic communications plan, one that has the potential to actually be used. A new contractor would be able to build these relationships and gain the institutional knowledge ; However it would take significant time and it would negatively impact the ability to meet the required delivery time of the outcomes at the end of the third year of the project (September 28, 2013). The purpose of this purchase order is to provide continuation of management services and evaluation expertise and oversight to support an infection preventionist with knowledge and experience in hospital healthcare-associated infections (HAI) prevention, surveillance, and reporting in small, community, or rural hospitals. Background The recognition of healthcare-associated infections (HAI) as a significant public health problem, and efforts to prevent HAIs were initiated in the California Department of Public Health (CDPH) in the early 1990's. Three pieces of legislation (Senate Bill (SB) 739, Speier, Chapter 526, Statutes of 2006; SB 158, Flores, Chapter 294, Statutes of 2008; and SB 1058, Alquist, Chapter 296, Statutes of 2008) now support infrastructure for a formal HAI Program. The provisions of this legislation require CDPH to take specific statewide actions to protect against HAI and serve as a cornerstone for HAI prevention efforts in California. Further, funding from the American Recovery and Reinvestment Act (ARRA), awarded in 2009 through the Department of Health and Human Services, allowed an opportunity to accelerate HAI prevention efforts in California's acute care hospitals. Implementing an HAI Prevention Program in California continues to be particularly challenging given the state's population of nearly 37 million residents, large numbers of acute care hospitals (439), extensive geographic spread (over 164,000 square miles) and wide-ranging cultural diversity and settings. This has necessitated a decentralized approach by public health, and led to an environment in which communication, collaboration and teamwork are not only desirable but essential to HAI prevention efforts. Intensive efforts are needed to assess the needs of California's small, rural, and/or critical access hospitals and assist in their HAI prevention activities. For the 164 small and rural California hospitals, the HAI Program has: 1) Evaluated HAI prevention needs; 2) Collected and or developed tools and best practices to help meet HAI prevention needs; 3) Developed a model approach for providing support and disseminating of best practices to California's remote hospitals; 4) Capitalized on partnerships with local public health officials in each county for supporting acute care hospital HAI prevention efforts locally. 5) Initiated a statewide prevention collaborative with 3 distinct focused activities: a. C difficile infection prevention with an antimicrobial stewardship component; b. HAI prevention best practices for California's smallest hospitals; and c. CAUTI prevention using the nationally recognized "CUSP" methodology. Continuation of this project is essential to assure ongoing success of these project activities by supporting the existing infrastructure for HAI prevention collaborative activities focusing on geographically diverse small and rural hospitals. Specifically, infection prevention expertise in the rural and small hospital setting is being provided, with strong skills in program oversight and evaluation. It is the intent that the approach and outcomes resulting from this California project will be expanded to develop a Western regional HAI Prevention consortium in partnership with other states in Region IX, and beyond, facing similar challenges in providing HAI coordination and support to hospitals in remote, underserved areas. Statement of Work 1. Continue to provide support for a 0.5 FTE Infection Preventionist to work with the California Department of Public Health/Healthcare Associated Infection (CDPH/HAI) Liaison Program to: A. Provide expert HAI prevention, surveillance, evaluation and reporting consultation to CA hospitals and local public health departments. B. Increase the numbers of in-person site visits and telephone consultations to small, rural, & critical access hospitals and local public health agencies each quarter. C. Increase healthcare providers' use of HAI data to drive internal prevention efforts using National Healthcare Safety Network (NHSN) standards. (http://www.cdc.gov/nhsn/) D. Support efforts for public reporting of HAI data by assisting in the development of messages (such as a monthly newsletter or update to providers) to be tested with rural consumer audiences. E. Contribute to CDC's national HAI prevention targets. From 2010 baseline, demonstrate rural hospital progress toward 5-year HHS prevention targets: - Reduce CLABSIs (rate by 50%) - Achieve 100% compliance with CLIP bundle - Reduce hospital onset of CDI by 30% - Reduce incidence of MRSA BSI by 25% - Reduce deep and organ space SSI by 25% - Achieve 95% compliance with SCIP measures F. Provide HAI education to hospitals, IPs, and other infection prevention stakeholders - increase over time the number of presentations & educational programs to small and rural hospitals and healthcare provider groups. G. Promulgate best practices & support culture change through effective HAI prevention collaboratives: Assist in the establishment and support of 1 or more small and rural hospital HAI prevention collaboratives (10 or more hospitals each) using evidence-based interventions. H. Seek input from the California HAI Advisory Committee: Support HAI-AC workgroups with a focus on specific HAI prevention needs of small and rural hospitals, providers, and consumers. Qualifications of Infection Preventionist (IP): The IP recruited and hired for this project must have (the current contractor meets all of these requirements): - A minimum of five years experience in hospital infection control. - Bachelor's degree in nursing or related field, or Master's degree in public health or epidemiology (Certification in Infection Control (CIC) is preferred). - A valid driver's license. - Skills and abilities that include: - Facilitating prevention collaboratives to support knowledge exchange and peer-to-peer learning. - Assisting hospitals in implementing infection prevention measures across varied clinical settings, including those in rural or small hospitals. - Evaluating infection prevention needs in a clinical setting, identifying areas that need improvement, and helping facilities identify interventions. - Working both independently and as part of a team comprised of individuals from different agencies, disciplines, and locations. - Communicating with diverse professional and lay audiences, and educating healthcare providers. - Documenting and tracking detailed project tasks, providing written summaries, interpreting findings. - Must have working knowledge of the National Healthcare Safety Network (NHSN). - Exceptional interpersonal skills and abilities. The position will require travel to regional hospitals with occasional overnight stays anticipated. Task 1: Continue to provide support to the California Department of Public Health/Healthcare Associated Infection (CDPH/HAI) Liaison Program. Contractor will provide ongoing support related to travel and office supplies for site visits. Task 2: On an ongoing basis, monitor, manage and expend all funding by the end of the project period. Task 3: Provide detailed on-time quarterly progress reports. Task 4: Develop and test methods for collecting, aggregating, and disseminating best practices for use by small and rural hospitals. Describe in detail the methods for collecting, aggregating and disseminating best practices in the Final Project Report. And the methods, skills and products will be packaged in a Model HAI Toolkit that can be provided to other states within Region IX as well as other regions, for how to effectively work with rural hospitals to improve infection control. Task 5: Develop, assess and evaluate an HAI prevention communication plan for its effectiveness with small and rural hospital healthcare providers, hospital leadership, local public health partners and rural community consumers. Task 6: Perform a data analysis to evaluate progress in HAI surveillance and reporting among small and rural hospitals. Demonstrate rural hospital progress toward 5-year HHS prevention targets from baseline established in 2010-2011 - Reduce CLABSIs by 50% - Achieve 100% compliance with CLIP bundle - Reduce hospital onset of CDI by 30% - Reduce incidence of MRSA BSI by 25% - Reduce deep and organ space SSI by 25% Achieve 95% compliance with SCIP measures. Task 7: Provide final summary report. Timeline: Ongoing through September 2013 - Continue to provide expert consultation to CA hospitals and local public health departments. Re-assess hospital HAI practices through collaborative projects and onsite visits. Increase knowledge, skills, and abilities to prevent HAIs at local level. Ongoing - Continue to exercise responsibility for oversight of the HAI prevention collaboratives, involving groups of hospitals and healthcare providers who are actively working on C difficile infection prevention and improved antimicrobial stewardship, HAI prevention best practices in very smallest hospitals (<30 beds), and "On the CUSP: Stop CAUTI." By beginning April 2013 - Perform analysis of data to evaluate progress in HAI surveillance and reporting among small and rural hospitals. By beginning June 2013 - Repeat evaluation of the effectiveness of outreach and communication activities. By mid September 2013- Develop report on the potential of this project to be used as a "model program" implemented in other Region IX rural hospitals and communities. Items to be Delivered and Delivery Schedule: 1. Provide detailed on-time Quarterly Progress Reports to include: a. Number of consultations to small, rural, & critical access hospitals and local public health agencies with a description of issues and their resolution b. Number of presentations & educational programs to small and rural hospitals and healthcare provider groups c. Quantitative results of efforts to increase healthcare providers' use of HAI data to drive internal prevention efforts d. Progress toward meeting HHS prevention targets with quantitative results: reduction of CLABSIs, reduction of hospital onset of CDI; reduction of incidence of MRSA; reduction of deep and organ space SSI, and progress towards achievement of compliance with CLIP bundle and SCIP measures 2. Contractor is expected to work closely with the California Department of Public Health HAI Program and attend California HAI Advisory Committee (HAI-AC) scheduled meetings where small and rural hospital prevention progress is being discussed. He/she will take the lead in seeking input from, and supporting an HAI-AC workgroup, to identify and address the specific HAI prevention needs of small and rural hospitals, providers, and consumers. Ongoing. 3. Final plan for communication messaging for small and rural hospital healthcare providers, hospital leadership, local public health partners, academia and rural community consumers. Draft due January 15, 2013. Government will review and provide comments by January 31, 2013. Final due February 15, 2013. 4. Data analysis to evaluate progress in HAI surveillance and reporting among small and rural hospitals. Establish baselines and quarterly targets. Due March 29, 2013. 5. Final report to summarize major project accomplishments, including documentation of the contractor's role in the support of and success of 3 targeted projects in the statewide small and rural hospital HAI prevention collaboratives, maintaining a minimum of 12 or more hospitals each), and documentation of methods for collecting, aggregating, and disseminating best practices for use by small and rural hospitals. Draft due August 30. Government will review and provide comments by September 13, 2013. Final due September 30, 2013. 6. Model HAI Toolkit that includes the methods, skills and products developed during this project. Toolkit will be packaged so that it can be provided to other states within Region IX, as well as other regions, to enhance their ability to effectively work with rural hospitals to improve infection control. There is no Government furnished equipment as part of this purchase order. The Contractor will independently and not as an agent of the Government furnish all necessary personnel, services, labor, materials, and equipment to perform the work. The Contractor will be directly responsible for ensuring the accuracy, timeliness and completion of all task specified in this Contract. In addition, all deliverables will comply with Departmental and government guidelines, standards and regulations (www.cdc.gov/nhsn). At least 30 days prior to diverting any of the specified individuals to other programs or contracts (or as soon as possible, if an individual must be replaced, for example, as a result of leaving the employ of the Contractor), the Contractor shall notify the Contracting Officer and shall submit comprehensive justification for the diversion or replacement request (including proposed substitutions for key personnel) to permit evaluation by the Government of the impact on performance under this purchase order. The Contractor shall not divert or otherwise replace any key personnel without the written consent of the Contracting Officer. The Government may modify the purchase order to add or delete key personnel at the request of the contractor or Government. (End of clause) Invoice Submission The Contractor shall submit invoices once per month. A complete invoice with all required back-up documentation shall be sent electronically, via email, to: 1. Invoices Only. The subject line of your email invoice submission shall contain the contract number, contract line item number, the order number, if applicable, and the number of invoices. The Contractor shall send one email per contract per month. The email may have multiple invoices for the contract. Invoices must be in the following formats: PDF, TIFF, or Word. No Excel formats will be accepted. The electronic file cannot contain multiple invoices; example, 10 invoices requires 10 separate files (PDF or TIFF or Word). Invoices shall be submitted in accordance with the contract terms, i.e. payment schedule, progress payments, partial payments, deliverables, etc. All calls concerning contract payment shall be directed to the COR. In accordance with FAR 52.212-4, Contract Terms and Conditions-Commercial Items, a proper invoice must include the following items: FAR 52.212-4(g) Invoice. (1) The Contractor shall submit an original invoice and three copies (or electronic invoice, if authorized) to the address designated in the contract to receive invoices. An invoice must include- (i) Name and address of the Contractor; (ii) Invoice date and number; (iii) Contract number, contract line item number and, if applicable, the order number; (iv) Description, quantity, unit of measure, unit price and extended price of the items delivered; (v) Shipping number and date of shipment, including the bill of lading number and weight of shipment if shipped on Government bill of lading; (vi) Terms of any discount for prompt payment offered; (vii) Name and address of official to whom payment is to be sent; (viii) Name, title, and phone number of person to notify in event of defective invoice; and (ix) Taxpayer Identification Number (TIN). The Contractor shall include its TIN on the invoice only if required elsewhere in this contract. (x) Electronic funds transfer (EFT) banking information. (A) The Contractor shall include EFT banking information on the invoice only if required elsewhere in this contract. (B) If EFT banking information is not required to be on the invoice, in order for the invoice to be a proper invoice, the Contractor shall have submitted correct EFT banking information in accordance with the applicable solicitation provision, contract clause (e.g., 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration, or 52.232-34, Payment by Electronic Funds Transfer- Other Than Central Contractor Registration), or applicable agency procedures. (C) EFT banking information is not required if the Government waived the requirement to pay by EFT. (2) Invoices will be handled in accordance with the Prompt Payment Act (31 U.S.C. 3903) and Office of Management and Budget (OMB) prompt payment regulations at 5 CFR Part 1315. Additionally, the FMS requires the contractor to include its Dunn and Bradstreet Number (DUNS) on each invoice. In accordance with OMB Memorandum, M-11-32, Agencies shall make payments to small businesses as soon as practicable, with the goal of making payments within 15 days of receipt of a proper invoice. If a small business contractor is not paid within this (15 day) accelerated period, the contractor will not be given a late-payment interest penalty. Interest penalties, as prescribed by the Prompt Payment Act, remain unchanged by means of this memorandum. All small businesses shall label all invoices as "Small Business." Additionally, in accordance with OMB Memorandum, M-12-16, all prime contractors are encouraged to disburse funds received from the Federal Government to their small business subcontractors in a prompt manner. To assist prime contractors in expediting contractor payments to small business subcontractors, Agencies shall, to the full extent permitted by law, temporarily establish an earlier, accelerated date for making agency payments to all prime contractors. Consistent with OMB Memorandum M-11-32 above, Agencies shall have a goal of paying all prime contractors within 15 days of receiving proper documentation. In an effort to support small business growth, drive economic activity and job creation, the Contractor is encouraged to accelerate payments to their small business subcontractors. In accordance with the requirements of the Debt Collection Improvement Act of 1996, all payments under this order will be made by electronic funds transfer (EFT). The Contractor shall provide financial institution information to the Finance Office designated above in accordance with FAR 52.232-33 Payment by Electronic Funds Transfer - Central Contractor Registration. This notice of intent is for information purposes only. All sources eligible to provide this service must respond in writing. Responses must be supported with clear and convincing evidence to clearly articulate the ability to provide services outlined above. Responses must include the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), certification of business size, and must have a valid and current Central Contractor Registration (CCR), and Online Representations and Certifications Applications (ORCA). A request for documentation will not be considered as an affirmative response. If no responses are received, by 7:00am on August 6, 2012, the effect that comparable services are available, the purchase order will be awarded without further notice.
