Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA251-12-P-0345 is issued as a request for proposals (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. This procurement is set aside for small businesses. The North American Industry Classification System (NAICS) number is 423450 and the business size standard is 100 employees. Requestors shall state their business size on their proposal. Proposed items must be BRAND NAME OR EQUAL to the items requested below, as they must be compatible with VistA, an existing VA computer program. All responsible sources may submit a proposal which shall be considered by the contracting officer. Requested Items: Line Item 1: One (1) Forum starter package including 1 archive, 1 viewer, interface and license (Item Code 266002-1141-610) Line Item 2: One (1) Cirrus 6.0.1 Central Archive Package including FORUM Archive with 2 instrument connections and remote installation (Item code 266002-1141-525) Line Item 3: One (1) Zeiss/Cirrus Review Software 5.2 with local database (20 pack) with remote installation (Item code 306601-8512-401-1 Line Item 4: One (1) Cirrus HD-OCT (Model 4000) Version 6.0.1 with all prior licensed features, advanced RPE, EDI, GCA and anterior segment, printer and table (Item code 266002-1146-356) Line Item 5: One (1) Cirrus 4000 - 4 year POS (extended) Items shall be delivered 30 days ARO to the Indianapolis VA Medical Center at 1481 W. 10th St., Indianapolis, IN 46202 The provision at 52.212-1, Instructions to Offerors-Commercial applies to this acquisition and offerors must comply with all instructions contained within it. Award will be made to the lowest price, technically acceptable offer. The following additional provisions and clauses apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4, 8, 12,, 23, 26, 27, 28, 29, 30, 31, 32, 38, 40, 42, and 48.. The following additional clauses apply, and upon request, the contracting officer will make their full text available: FAR 52.204-7, FAR 52.225-25, FAR 52.252-2, and VAAR 852.270-4 Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
