This RFP does not commit ResCare to pay any costs incurred in the preparation of a proposal, nor does it commit ResCare. to award a contract for products, supplies or services. ResCare will be the sole judge as to which bid offers the best overall value and will serve the best interest of ResCare. In compliance with the Bid Provision, Scope of Work, Terms and Conditions, and other specifications contained in this RFP, the undersigned hereby proposes to provide Mental Health Consultant Services for the center listed above. Bid Rate (Hourly Rate) The hourly rates herein are no higher than would be quoted to any other customer, either commercial and/or agencies of the United States Government, for similar services. Mental Health Services Base Term August 31, 2012 through July 31, 2013 1 st Option Year August 1, 2013 through July 31, 2014 2 nd Option Year August 1, 2014 through July 31, 2015 3 rd Option Year August 1, 2016 through July 31, 2017 Hourly Rate Above rates include necessary labor, transportation and supervision required to accomplish scope of work. Bidders must include proposed rate for base and option years, as applicable. •· Valid licenses to practice in the state or commonwealth •· Valid DEA license (see DEA License section under Terms and Conditions) •· Certificate of insurance (see Insurance section under Terms and Conditions) •· Resume or Curriculum Vitae •· Statement of Experience dealing with adolescents •· Proposed schedule (days/hours) •· Individual Hourly Rate Breakdown form, if applicable SELECTION OF CONTRACTOR In addition to the hourly rate of service, selection of the contractor will be based on the following criteria: •· Experience in providing mental health care for adolescents and young adults (minimum of five years experience) •· Provision of mental health care to young adults who are socially and economically disadvantaged •· Previous practice in settings such as schools, colleges, institutions, military or correctional institutions •· Credentials to supervise Master level candidates for internship at the Pinellas County Job Corps Center •· Willingness and ability to carry out required duties and responsibilities •· Admission and attending privileges at local hospitals or specialty clinics •· Academic affiliation with nearby professional schools or colleges •· Approval of the contractor by the ResCare corporate office •· Approval of the contractor by the Regional Office of Job Corps, U.S. Department of Labor The most suitable candidate(s) may be interviewed prior to award. BID PROVISIONS The following bid provisions will apply. Failure to carefully read and follow all instructions and specifications will be at the bidder's risk. 1. All prices and notations must be in ink or typewritten. No erasures are permitted. Errors may be crossed out and corrections printed in ink or typed adjacent and must be initialed in ink by person signing bid. 2. Price Warranty: The hourly rates quoted herein are no higher than would be quoted to any other customer, commercial and/or agencies or the United State Government for similar services. 3. The bidder will list on a separate sheet of paper any exceptions to the conditions of this bid. This sheet will be labeled "Exceptions to Bid Conditions" and will be attached to the Bid Sheet. If no exceptions are stated, it will be understood that all general and specific conditions will be complied with without exception. 4. ResCare reserves the right to accept and/or reject any or all bids or waive any formality connected with the bids. Lowest rate or overall bid does not constitute bid award. ResCare will be the sole judge of which bid is selected for approval (see "Selection of Contractor" above) and contract award. 5. Items will be quoted F.O.B. destination. No additional charge for delivery, drayage, express, parcel post, packing, cartage, insurance, license fees, permits, cost of bonds, or for any other purpose will be paid by ResCare unless specifically included in the bid and accepted by ResCare. 6. In the event that a contract is awarded in response to this RFP, the below statement of work, provisions and set forth terms and conditions will be incorporated in their entirety into the contract and become an integral part of the contract when the contract is accepted by the contractor, either by acknowledgment or commencement of performance. SCOPE OF SERVICES The undersigned center mental health professional agrees to provide students of the Pinellas County Job Corps Center with the services stated herein. Such services will be provided as authorized by the Center Director or his/her representative. The Center and the Mental Health Consultant hereby agree that the services to be provided will include, but will not be limited to those mental health services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510; the Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.10, R3; and Technical Assistance Guide D: Center Mental Health and Wellness Program). The services to be provided include: A. Staff Development (minimum 1 hour per 100 students per week): •Þ Advice to the Center Director and health and wellness staff on all mental health matters and overall Center program and organizational consultation to the Center Director and administrative staff on a regular basis. This will include staffing requirements. •Þ Consultation on the development of a system to promote the coordination and integration of the mental health and wellness program with other Center programs and activities such as counseling, residential life, and recreation. This will be done in cooperation with the Health and Wellness Manager, Center physician, Center dentist, other health professionals, as well as any staff member involved with students. •Þ Advice and assistance to wellness course instructors regarding mental health and wellness services during student introduction to Center life. •Þ Consultation with Center health and wellness staff, counselors, residential advisors, instructors and other appropriate staff concerning the ongoing mental health problems of students. •Þ Assistance with program development for the purpose of meeting the life crises of students in constructive ways. For example, involvement with the career preparation, development, and transition periods, the disciplinary program, the maximum benefits program, the cultural awareness program, and the trainee employee assistance program (TEAP). •Þ Staff development on using mental health principles and techniques. This will include coping with problems of alcohol and drug use, sexuality, aggressive behavior and poor communications. B. Clinical Services (minimum 3 hour per 100 students per week): •Þ Advice to the Center Director on the advisability of students' separations for mental health reasons, and the cost of mental health services, including inpatient facility use. •Þ Ensure the maintenance, confidentiality, and safeguarding of all necessary mental health records. •Þ Issuance and maintenance of signed and dated health care guidelines regarding mental health matters. •Þ Diagnostic and evaluative services to students who are inpatients or outpatients. •Þ Limited forms of psychiatric treatment including crisis intervention, brief psychotherapy, intermittent supportive psychotherapy, psychotropic drug management and group psychotherapy, as permitted by licensure or accreditation. •Þ Assisting with appropriate referral for students who must be separated for mental health reasons. C. TEAP Consultation Services (minimum 1 hour per 100 students per week): •Þ Consultation on the development of a system to promote the coordination and integration of the mental health and wellness program with the TEAP and related centerwide programs •Þ Staff development on using mental health principles and techniques, including coping with problems of alcohol and drug use. •Þ Providing clinical assistance to students and the TEAP Specialist, especially with students who may have co-morbid psychiatric and substance abuse disorders. D. Review folders and assist with the reasonable accommodation process. (This service should be contracted at additional hours as deemed appropriate by the center.) The Mental Health Consultant agrees to adhere to the policies, procedures, program instructions, and guidelines established by Job Corps. The Mental Health Consultant and the Center further agree that the Mental Health Consultant will have primary responsibility under the Center Director's guidance to implement the program outlined above. The Mental Health Consultant, with the concurrence of the Center Director, may delegate by written and dated health care guidelines any of his/her responsibilities set forth in this subcontract to appropriate qualified Center staff. The Mental Health Consultant will be assisted by the Job Corps National Office, principal mental health consultant, and regional health consultants. The latter will act under guidance and direction from the National Office of Job Corps. OBLIGATIONS OF THE MENTAL HEALTH PROFESSIONAL In the performance of the above duties and responsibilities, the Mental Health Consultant: •A. May not delegate professional responsibilities set forth in this contract to another full or part-time physician, except upon prior written concurrence of the Center Director and prior review and approval by the Job Corps Regional Director and Regional Health Consultant. •B. Understands and agrees that the medical services under this Subcontract are subject to monitoring and review by the U.S. Department of Labor, Office of Job Corps. C. Agrees to maintain malpractice insurance to cover the professional services to students, with limits consistent with the medical service industry, but in no case less than $1 million per occurrence. D. Agrees to make no public statements regarding its activities under this Subcontract, or to disclose any information regarding Job Corps students or Job Corps activities without prior written approval of the Center Director or his/her designated representative. Unauthorized release of student records is a federal crime punishable by fine and/or imprisonment. E. Shall be responsible for finding and arranging for a professionally qualified replacement/substitute during temporary periods of absence of more than one calendar week. F. Shall be personally and financially liable for the cost of any patient referral outside the Center without prior written approval of the Center Director. G. Certifies its intent to comply with all equal employment opportunity practices and affirmative action policies as required by law and agrees not to discriminate against any student, employee and/or contractor because of race, religion, color, gender, age or national origin. •H. Shall provide current copies of all professional licenses, certifications and professional liability insurance documents required by the Center. •I. Agrees that all research projects and/or publications relating to Job Corps health activities will first be cleared through the National Office of Job Corps, Washington, D.C. •J. The Mental Health Consultant will submit an invoice at the end of each week for which services have been provided, approved by the Wellness Services Manager. The invoice must include the number of hours worked each day of service. •K. Mental health consultant is required to comply with the Center's sign-in procedures, a log book containing sign-in sheet is kept in the health and wellness department and is updated and filed on weekly basis OBLIGATIONS OF THE CENTER To support the Mental Health Consultant in the performance of its duties under this Subcontract, the Center shall: A. Provide all supplies, materials and services, including facilities and equipment, as well as adequate levels of qualified staff required to operate the on-Center Wellness Services Clinic. B. Provide all U.S. Department of Labor, Office of Job Corps guidelines, handbooks, etc., required to operate the Job Corps Wellness Services Program.
