- Questions and answers about Disease Manager dated 27 Jul 12 Q-1: Would Yokota Air Force Base consider full or partial reimbursement of the airfare necessary to transport the Disease Manager to and from the US? Answer: The Government will not provide any equipment or services other than those items specifically noted in Paragraph 3 of the PWS. Vendors are urged to factor all costs associated with performance and include them in their quote. Q-2: What documents are needed to submit a bid ? Answer: Paragraph (ii) on the 2nd page of the solicitation describes as below. The offeror is requested to submit the following for assessment of technical acceptability in accordance with Paragraph 1.2 of the PWS: 1. Degree or Diploma from a professional nursing program. 2. An active unrestricted Nursing license. 3. Certification of current physical examination. Q-3: Should we also submit the candidate's resume? Answer: It is not mandatory to submit a resume. Q-4: On the Price Schedule, in the "REMITTANCE / EFT ADDRESS" area, should we write the physical address of our bank? Or the Account number? Or both? Answer: Please write only physical address of your bank.
