Description
Contractor to provide Radiopharmaceutical products as listed herein, as requested by authorized VA Staff, in accordance with Public Law 104-262 and 38 USC 8153 in order to support the Amarillo VA Healthcare System, 6010 Amarillo Boulevard West, 79106. The Government anticipates issuing a solicitation on or about 8/17/2012 through FedBizOpps.gov and all responsible sources may submit a bid, proposal, or quotation which shall be considered by the agency. A maximum of two (2) awards for a base year with one option year will be awarded. Base Year: 10/1/2012 - 9/30/2013 Option Year 1: 10/1/2013 - 9/30/2014 The anticipated award date is on or before 9/30/2012 for orders/deliveries to begin 10/1/2012. EMERGENCY DELIVERY FEES (FOR OTHER THAN ROUTINE SCHEDULED) 1EMERGENCY WEEKDAY/HOLIDAY DELIVERY TO BE MADE WITHIN 2 1/2 HR OF TELEPHONE REQUEST BETWEEN 0730 AND 1700150EACH WORK STATEMENT: 1.Dual Contractor shall furnish and deliver radiopharmaceutical supplies as listed above, F.O.B. Destination, to the Amarillo VA Healthcare System, 6010 Amarillo Boulevard West, Amarillo, Texas 79106. 2.Dual Contractor will provide for the placement of one or more telephone delivery order (s) for items listed on pages 2-4 per day, 24 hours per day, seven days a week, including holidays. 3.Contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing and transporting radiopharmaceuticals. 4.DELIVERY ORDERS: Normally orders shall be placed late in the work day and delivery shall be required by 7:00 am the following work day. Due to the demands of the Medical Center care requirements; there shall be no restriction to the number of orders which may be placed by VA, nor the number of delivers which may be required per day. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly. The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA order number. Orders can be placed orally by telephone and/or fax transmission. 5.DELIVERY LOCATION: Deliveries will be made to: Amarillo VA Healthcare System Attn: Nuclear Medicine Department Hot Lab, Room 1303 Building Twenty Eight, First Floor 6010 Amarillo Boulevard West Amarillo, Texas, 79106 6.EMERGENCY ORDERING: An added cost is provided in line item "D" for "emergency" orders, which require same day delivery. Depending upon the availability of the product, the Contractor shall make deliveries within 2 ½ hours when orders are placed between the hours of 7:30 am and 5:00 p.m. and within 2 ½ hours when orders are placed between the hours of 5:00 p.m. and 7:30 am.
