- Attachment #2 - Lake Creek Full Set of Drawings Solicitation Package - AG-84N8-S-12-0124 Attachment 1 - Lake Creek Culvert Supplemental Specifications Presolicitation Notice AG-84N8-S-12-0124 - Lake Creek Culvert Replacement on the Dixie National Forest located in Utah. This project includes the construction of a temporary bypass road around the existing culvert crossing, removal and disposal of an existing corrugated metal pipe culvert, the installation of a structural steel arch pipe with buried concrete footings plus associated roadway fill and stream restoration work, and removal /obliteration of the temporary bypass road once traffic is diverted back onto the original roadway at the Lake Creek stream crossing on Forest Road 30153, Milepost 29.473 on the Escalante Ranger District, as detailed below: The specific work at the Project Site (Forest Road 30153, M.P. 29.473) includes the following: Furnishing and maintaining appropriate traffic control and warning signs and hardware; Hauling material for and constructing a temporary bypass road to divert public traffic off of FR30153 during the project work; Constructing a temporary stream by-pass to divert the stream flow in Lake Creek during the project work; Removing and disposing of an existing 72-inch diameter by 44 ft. long pipe culvert; Removing and salvaging for re-use the existing roadway fill and riprap material around the pipe; Furnishing and casting reinforced concrete footings with stem walls for structural steel arch; Fabricating and installing an 8'-0" span, 3'-4" rise, 56 ft. long structural steel arch; backfilling and reconstructing the roadway fill approaches on both sides of the new culvert to the design finished grade; furnishing and placing a layer of select fill material within the new arch pipe footings to create a simulated natural stream course; and restoring 109 ft. of stream course including construction of 8 in-stream rock vane structures (with 4 of the structures built inside the pipe) to facilitate improved aquatic organism passage. The intent of the contract is to provide for the complete construction of the project described in the contract. Unless otherwise provided, the contractor is to furnish all labor, materials, equipment, tools, transportation, and supplies and perform all work required to complete the project in reasonably close conformity with the drawings and specifications, and in accordance with the provisions of the contract. Water sources are not designated. The Forest Service has no water rights available for use in the project area. The Contractor will be responsible for getting permission from the local irrigation company or landowners to use water adjacent to the project site, or will need to furnish water from other sources. The project is located on the Escalante Ranger District of the Dixie National Forest, Garfield County, Utah within Section 3, Township 33 S., Range 3 E., Salt Lake Meridian. It is located on National Forest Service Road 30153, 8.5 road miles northwest of Boulder, Utah and 10 road miles northwest of the junction of Utah State Route 12 and Salt Gulch Road/National Forest Service Road FR30153. To access the project from Boulder or Escalante, drive on Utah State Route 12 to near Milepost 82 to the junction with the Salt Gulch Road (signed as the Hells Backbone Loop Road). Turn onto the Salt Gulch Road and follow it to where it becomes Forest Road FR30153. Continue on FR30153 for 10 miles (measured from the junction with State Route 12) to a road Junction between FR30153 and FR30566; the project site is 95 feet west of the FR30153/FR30566 junction. The start date is anticipated to be summer to fall of 2012. The actual date for Notice to Proceed will be negotiated with 90 Calendar Days for completion. Presolicitation Notice will run through August 8, 2012 at which time a full solicitation package will be posted to this website. Quotes for this solicitation will be due on August 23, 2012 at 1:00 p.m. Local Time. Offerors are cautioned to continually monitor this website for any Amendments to the solicitation notice that may be posted. It is the offerors responsibility to take necessary steps to ascertain the nature and location of work and to investigate and satisfy themselves as to the general and local conditions that can affect the work or its costs. August 8, 2012 Posting of full Solicitation Package and Attachments. A site visit has been set for August 16, 2012 at 2:00 p.m. Offerors should plan to meet at the project site (reference Appendix #4, Lake Creek Access Map (located in solicitation).
