- Statement of Work-Hospital Beds/Stretcher 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation number RFQ12-PHX-09 is being issued as a Request for Proposal (RFP). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-57. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This solicitation is a total small business set-aside. The North American Industry Classification System (NAICS) code is 339111 - Furniture. 5. This requirement consists of three (3) line items: 0001 - Hospital Beds 0002 - Birthing Beds 0003 - Obstetrics/Gynecology Stretcher 6. The Statement of Work and specifications are attached to this notification. The requirement is for 6-Hospital Beds, 2-Birthing Beds and 1-OB/GYN Stretcher. 7. The location of contract delivery is Hopi Health Care Center, Highway 264, Mile Marker 388, Polacca, Arizona 86042, and the point of delivery is FOB Destination to the same address. The Delivery is 42 Calendar Days after issuance of Purchase Order. 8. INSTRUCTIONS TO OFFERORS: 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation. Offerors must submit a technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature or other documents necessary. Terms of any express warranty; price and any discount terms; past performance information; statement specifying the extent of agreement with all terms conditions and provisions included in the solicitation. Failure to furnish required information or reject the terms and conditions of the solicitation may be excluded from consideration. 9. PROVISIONS/CLAUSES: 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered: I. Price The evaluation factors are listed in descending order of importance.
