Description

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All bidders must also be registered on www.ccr.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II. This solicitation is issued as a request for quotation (RFQ). Submit e-mailed quotes on RFQ Number VA246-12-Q-3289. This solicitation is a request for Optical Coherence Tomography equipment with trade in of Stratus OCT which meets the requirements within this solicitation to be delivered to the VA Medical Center, Salem, VA. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. IV. This is an 100% Small Business set aside. The North American Industry Classification System Code is 423450 and the size standard is 100 employees. V. This combined solicitation/synopsis is for purchase of the following commercial item/s. The following item/s, brand name or equal to the 266002-1146-356 Metro Medical product, are requested. If offering equal, provide the manufacturer name, brand, part number, unit price and total cost. List trade in value for Stratus OCT. 1. Optical Coherence Tomography, Cirrus HD-OCT Model 4000, Version 5 w/OHN, OHN Norm Data, Anterior Segment, Printer and Table. Grand Total: VI. Required delivery within 30 days of award. Deliver all products FOB Destination to the Salem VAMC, 1970 Roanoke Blvd., Salem, VA 24153. VII. FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. VIII. The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for proposal, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: Technical and Price. a. Technical: ability to provide products which conform to the characteristics and requirements listed in this solicitation b. Price IX. The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2011), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. X. FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. XI. FAR clause 52.212-4, Contract Terms and Conditions - Commercial Items (Jun, 2010), applies to this acquisition. XII. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Apr, 2011) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.204-10, 52.209-6, 52.219-4, 52.219-4, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-3 Alternate I, 52.225-13, 52.232-34.
