- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number N68836-12-T-0352 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the latest edition of the Federal Acquisition Circular. This solicitation is issued on a total small business set a-side basis, NAICS Code 336611; The USS Philippine Sea (CG58) located at the Naval Station Mayport Florida 32228 requires habitability work as described in the Statement of Work: Statement of Work USS PHILIPPINE SEA (CG 58) Habitability Work Our ship has some serious material conditions that need to be corrected. Below is a list of jobs that we would like to be replaced and accomplished. I will provide you a tour so you can provide an estimate on what the jobs will cost. We will then submit that to contracting as a quote needed to get the contract awarded. 1)Refurbish and re-upholstery CMC sofa. 2)Replace cabinets throughout the Wardroom, Replace 26 chairs within the Wardroom. Install new coffee table. Install new plastic cabinets in the lounge. Replace wall lights. Install new plastic on cabinet in lounge. Replace chair rail Install new VT plastic around post in lounge. Repair all hardware on cabinets. 3)Manufacture a cabinet w/ two trash cans inside. Manufacture Milk cabinet lock. Reupholster sofas. Replace two each missing latches on cabinets. Govt. This certificate shall originate and be signed by the manufacturer of the material. In no case shall the Government's right to inspect supplies under the inspection provision of this contract be prejudiced. The signed certificate shall be attached to or included on the top copy of the DD250, DD1155, or SF1449 copies distributed to the payment office. When acceptance is at destination, a copy of the signed certificate shall also be attached to or entered on copies of the DD250, DD1155, SF1449 or other appropriate documents accompanying the shipment. This certificate shall read as follows: I hereby certify that on _________(date), (Contractor s name) provided the supplies called for by contract number ________________ via ____________________(carrier) on ________________(bill of lading or shipping document) in accordance with all applicable requirements for shipment. I further certify that the supplies are of the quality specified and are in all respects in conformance with the contract requirements, including MIL-STD-1623 (SHIPS) and other specifications, preservations, packaging, packing and marking requirements; physical item identification (part number), and in the quantity shown on this or the attached acceptance document.
