- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a solicitation W91YU0-12-T-0147A, 0010194760 issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60 The associated North American Industrial Classification System (NAICS) code for this procurement is 424210. Size Standard for small business is 100 employees. RFQ is being issued as 100% Small Business Set-aside. This requirement is for Various Pharmaceuticals to be used at William Beaumont Army Medical Center and the proposed contract is for a one-time buy for a required delivery date of 1 October 2012 thru 30 September 2013. The pharmaceuticals products will be requested on an "AS NEEDED BASIS by the Government point of contact. THE CONTRACT WILL BE ISSUED IAW FAR CLAUSE 52.232-18 "AVAILABILITY OF FUNDS". A MODIFICATION WILL BE ISSUED WITH THE FUNDING ONCE IT BECOMES AVAILABLE All responsible Contractors shall provide an offer for the following Brand Name or Equal Products: CLIN 0001: Fentanyl Citrate 10 mcg/ml 5 bags per case: qty 180; unit issue: Case CLIN 0002: Midazolam HCL 1mg/ml 5 bags per case, qty 200; unit issue: case CLIN 0003: Heparin 1u/ml 500ml in NS 12 bags/ case, qty: 120; unit issue: Case CLIN 0004: Oxytocin 20u/ml 1000ml 6 bags/case; qty; 200; unit issue: Case CLIN 0005: Hydromorophone 0.2 bags/case;qty: 200; unit issue: case CLIN 0006: Phenylephrine 100mcg20u 5 bags/case;qty: 360; unit issue: case CLIN 0007: Ropivicaine 0.2%/.9%nacl 600ml/;qty: 160;unit issue: case CLIN 0008: Freight Charges (for the entire period of performance) Pricing quote shall include all applicable fees.
