Description
REQUEST FOR INFORMATION, SOURCES SOUGHT THIS IS A REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT ONLY AND YOUR RESPONSE IS NOT AN OFFER AND DOES NOT COMMIT THE GOVERNMENT TO PAY ANY COSTS INCURRED IN PREPARATION OF ANY SUBMISSION TO THIS MARKET RESEARCH, OR TO CONTRACT FOR SUPPLIES/SERVICES. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to obtain information regarding the availability and capability of qualified small business sources. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. This will be a split award FFP Indefinite Delivery Indefinite Quantity (IDIQ) contract with base and four, one-year ordering periods. The intent of this notice is to determine the availability of qualified commercial sources technically capable of providing the items below: REQUIREMENTS: Veterans Integrated Service Network (VISN) 19 is seeking vendors to supply blood products in FDA approved plastic blood bags appropriate for each blood product for the Department of Veterans Affairs Hospital Cheyenne, Wyoming (VAMC), VA Montana Healthcare System Ft Harrison VAMC, and George E. Wahlen SLC VA Health Care System, Grand Junction Veterans Affairs Medical Center. Product shall be supplied in such quantities and at such times as may be required in accordance with all terms, conditions, provisions and the schedule. The products shall include packed red blood cells, irridiated packed red blood cells, washed or deglycerolized products, fresh frozen plasma, cryoprecipitate, platelets and other blood products as agreed on by client and vendor; as well as reference lab testing including Antibody ID, Antibody screening of units, Crossmatch, DAT, ABO discrepancy, Rh Discrepancy, Transfusion Reaction, Elution, Incompatible crossmatch, Antibody titer, Molecular genotyping and other as agreed on by the client and contractor. All interested firms who can meet the requirements stated above should respond, in writing, with a capability statement describing their interest, capabilities, geographic location and indicating the business category in which the firm belongs. The capability statement should be sent to the Contract Specialist Kim Cooper no later than 1:00 p.m. (Mountain Time), 22 Aug 2012, via e-mail to Kim.Cooper3@va.gov. RESPONSES SHOULD INCLUDE THE FOLLOWING INFORMATION: company name, address, point of contact, phone number, e-mail address and DUNS/CAGE code. Any questions or concerns may also be directed to Kim Cooper via e-mail. Solicitation VA259-12-R-0207 Blood Products and related services have been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 621991 is applicable to this acquisition; the size standard for a firm to be considered under this NAICS Code is a business size of $10 million. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
