Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6 as supplemented with additional information included in this notice. This is a Service-Disabled Veteran-Owned Small Business Set-Aside requirement. Only qualified offerors may submit bids. Solicitation VA-249-12-Q-0844 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. NAICS Code is 541618 and size standard is $7.0M. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. Wage determination WD 2005-2499 Revision 13 dated 06/19/2012 applies to this request and can be accessed at the below link: http://www.wdol.gov/wdo/scafiles/std In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of furnishing all labor, material, and services required for the Confined Spaces Entry Program at the James H. Quillen - Mountain Home VA Medical Center. Period of Performance (POP): The base POP is from September 1, 2012 through November 9, 2012. Requirements: The vendor will gather information and develop an inventory of VAMC Permit-Required and Non-Permit-Required Confined Spaces as defined in 29 CFR 1910.146, that the vendor will provide to the VAMC in a searchable database (Excel) capable of being used for tracking of Confined Space Entries and changing conditions. The vendor will develop and OSHA-compliant Confined Space Entry Evaluation Form that is also compatible with standards and a Confined Space Permit format for use by the VAMC. The vendor will conduct an evaluation of each space to include the hazards present and document the evaluation using photographs that can be placed by the VAMC into a Confined Space Inventory management program using SharePoint Software. Information to be provided by vendor will include: pictures that show (through arrows, highlighting, etc.) the location of the identified Confined Spaces evaluated under this contract, list of hazards present in the confined space, unusual characteristic of the confined space, and any other relevant information needed to adequately characterize the confined space for risk assessment. Examples of types of Confined Spaces to be evaluated include (but not limited to) all Manholes Tanks, Pits, Utility Vaults, Storage Bins, Hoppers, Pipeline Pumping Stations, Reaction or Process Vessels, Sewer Storage Tanks and Vats. VAMC Mountain Home will furnish the following: (1) VAMC shall provide escort and/or access to all mechanical areas of buildings. (2) Written permission to have and use camera for taking pictures of equipment to be used for documenting evaluations of confined spaces, (3) Drawings that show the location of manholes and the utilities they contain, (4) Numbering system for manholes, (5) Proper manhole opening tool, (6) Assistance with traffic control for access to manholes that must be entered in the street. Contract Type: The government anticipates awarding a Firm Fixed Price award. To receive consideration as SDVOSB or VOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Award will be made to the lowest price quote which conforms to the requirements within this solicitation. NOTE: If claiming SDVOSB, firm must be registered and CVE verified in VetBiz Registry www.vetbiz.gov.
