Attachment F - HHS Section 508 Evaluation Template Attachment E -OMB SF--LLL Dislosure of Lobbying Activities Attachment D - Non-Disclosue Agreement Attachment C - Past Performance Evaluation Survey Attachment B - Subcontracint Plan Attachment A Wage Determination Solicitation 12-233-SOL-00557 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. The Solicitation number is 12-233-SOL-00557 and is being issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58, effective May 18, 2012. Offerors should be registered in the Central Contractor Registration (CCR) database. The North American Industry Classification (NAICS) Code is 524298. The small business size standard is $7M. A Fixed Price contract is anticipated The USPHS Active Duty Dental Program seeks a service provider with a large accessible national network of at least 65,000 dental providers for their Active Duty Commissioned Corps Officers. The service provider will be required to administer dental benefits for Active Duty USPHS Commissioned Officers throughout the world. The provider must have the financial capability to pay all dental claims weekly (approximately $75,000-$90,000) and be reimbursed up to 30-35 days later. Up to $360.000 may be paid out each month. In addition, the provider's services for the predetermination processing, and claims adjudication and payment will be paid under separate invoice on a monthly or quarterly basis. Any providers determined not to have the financial capability will not be considered further. The selected provider shall have an interactive website for beneficiaries and providers that are available to all Active Duty USPHS Dental Program customers. Services online to include: beneficiary access to information about their dental benefits and claim status; provide online patient benefits to dental providers, which shall include patient eligibility, benefits, claim status and payment details; shall allow providers online electronic claims submission; and provide an online link to search and get directions to a network provider. These online services are to be compliant with Section 508 of the Rehabilitation Act of 1873 (29 U.S.C. 794). The selected provider must be able to provide section 508 compliant websites This acquisition is being conducted using Part 12 of the Federal Acquisition Regulation. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Provision at 52.212-2, Evaluation-Commercial Items, is applicable. Evaluation of proposals will be conducted in accordance with the criteria identified under the solicitation. Refer to the attached Solicitation for the specific evaluation criteria. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Refer to attached solicitation for further details. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Refer to attached solicitation for further details. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. Refer to attached solicitation for further details. Please read the attached solicitation carefully and prepare your proposal in accordance with the solicitation requirements. It is the Offeror's responsibility to submit all questions and proposals by the scheduled closing date and times, in accordance with the "Instructions to Offerors". Please note that the deadline for questions is August 29, 2012 by 3:00pm Eastern Time. All questions must be submitted via email. All responses will be posted on the Federal Business Opportunities website. Proposals shall be submitted no later than September 13, 2012 by 3:00 PM Eastern Time. This RFP does not commit the Government to pay any costs for the preparation and submission of your proposal. It is also brought to your attention that the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with the proposed requirement. The Government reserves the right to award a contract without discussions if the Contracting Officer determines that the initial offer(s) is /are providing the best value and discussions are not necessary. It is the Offeror's responsibility to monitor the FEDBIZOPPS website for the release of any amendments to this solicitation. No telephone or faxed requests for the solicitation will be accepted. The period of performance has a base period of 12 months with four (4) 12 months option periods.
