- (i) This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request for Quote (RFQ) F1SCOM21930200 is a combined synopsis/solicitation for commercial service. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. Central Contractor Registration (CCR) is only required from individuals responding to the solicitation who are incorporated in the US. (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60 and through Department of Defense Acquisition Regulation Change Notice 20120724. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm (iv) This procurement is being issued unrestricted. North American Industrial Classification Standard: 517110. Small Business size standard: 1500 employees. (v) Fill out the following bid schedule. All prices shall be in Kuwaiti Dinar (KWD). Any contractor that proposes prices not on the following bid schedule will be determined non responsive. CLIN 0001 - 155MB internet service. (Base, Year 1) Contractor must be able to provide at least 155 MB of wireless internet service Ali Al Salem Air Base, Kuwait in accordance with the Statement of Work.* POP 25 Sep 2012 to 24 Sep 2013. 12 Months x ___________(Unit Price)= _____________ (Extended Price) CLIN 1001 - 155MB internet Service. (Option 1, Year 2) Contractor must be able to provide at least 155 MB of wireless internet service to Ali Al Salem Air Base, Kuwait in accordance with the Statement of Work.* POP 25 Sep 2013 to 24 Sep 2014. 12 Months x ___________(Unit Price)= _____________ (Extended Price) CLIN 2001 - 155MB internet Service. (Option 2, Year 3) Contractor must be able to provide at least 155 MB of wireless internet service to Ali Al Salem Air Base, Kuwait in accordance with the Statement of Work.* POP 25 Sep 2014 to 24 Sep 2015. 12 Months x ___________(Unit Price)= _____________ (Extended Price) *See attachment 1 for Statement of Work. (vi) Performance at: Ali Al Salem Air Base, Kuwait. Responsibility and Inspection: Unless otherwise specified in the contract or purchase order, the supplier is responsible for the performance of all inspection requirements and Quality Control (QC). (vii) The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be the most advantageous to the government based on price. Award shall be based on All or None. (viii) The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. FAR 52.212-4 addendum is also included. Additionally, the Contractor shall reference the contract number, as identified within Block 2, for all invoices submitted. Furthermore, all invoices submitted shall be Clean, Legible, and in English.
