Description

(i.)This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Additionally, this announcement constitutes the only solicitation for The Fargo VA Health Care System - Health Care for Homeless Veterans (HCHV); The Health Care for Homeless Veterans is seeking proposals from interested vendors to award either a single or multiple contracts for each community identified in Table A - Locations & Point of Contact Information for Interested Parties. Interested Temporary Housing/Homeless Shelter should review paragraph (v.) of this combined synopsis / solicitation for instructions on how to initiate a request to participate in the Fargo VA Health Care System - Health Care for Homeless Veterans (HCHV) Program. (ii.)The reference / solicitation number is VA263-12-R-1110 and the solicitation is issued as a Request for Proposals (RFP). The combined synopsis / solicitation is being issued as an unrestricted Request for Proposals (RFP) by the Fargo VA Health Care System - Health Care for Homeless Veterans under statutory Authority Title 38, U.S.C. §501 and §2031 (a) (2). The Government anticipates entering into acquisition negotiations and award of a single or multiple firm-fix-price (FFP) indefinite-delivery indefinite-quantity (IDIQ) contract for a base year and four (4) renewable option years for the placement of eligible veterans into regional (community-based) temporary housing/homeless shelters in the following locations: Table A - Locations & Point of Contact Information for Interested Parties CountyStateFargo VA Health Care System Contract Officer Information KittsonMNName: Phone Number: Fax Number: Email Address:John D. The temporary housing / homeless shelter shall to provide non-VA community-based facilities that provide temporary housing, as well as care and treatment and/or rehabilitative services to homeless Veterans (female and/or male) who are seriously mentally ill or have a substance use disorder in accordance with the terms and conditions stated herein, to furnish to and for the Fargo Veterans Affairs Health Care System, Health Care for Homeless Veterans program, the services and prices specified in the Section entitled Schedule of Supplies/Services of this contract. (iii.)The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication through Federal Acquisition Circular 2005-53. Provisions and clauses incorporated by reference have the same force and effects as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv.)North American Industry Classification System (NAICS) Code is 624221 with the small business size standard of $7.0 million (USD) applies to this solicitation. (v.)The only contract line items for this procurement would be based on the type of rooms the potential contractor provides temporary (homeless) shelter to eligible veterans. (vi.)This requirement is for community based Temporary Housing / Homeless Shelters for eligible veterans. (vii.)The Fargo VA Health Care System - Health Care for Homeless Veterans anticipates award of a single or multiple firm-fix-price (FFP) indefinite-delivery indefinite-quantity (IDIQ) contract (at least one to provide this service to these counties) to the responsive / responsible offeror(s) whose offeror conforming to the solicitation will be most advantageous to the Government per Federal Acquisition Regulation (FAR) Part 13.106-2 for a period not to exceed 12-months, with four (4) one-year option periods, at the discretion of the Government within prescribed time periods on or before September 21, 2012. The Government reserves the right not to award an indefinite-delivery indefinite-quantity contract if it determines that it is not in the "best interest" of the Government. (viii.)The provision at Federal Acquisition Regulation (FAR) 52.212-1 Instructions to Offerors - Commercial Items (June 2008), applies to this solicitation and incorporate the following addenda to 52.212-1; 52.204-6 Data Universal Numbering System (DUNS) Number; 52.212-4 Contract Terms and Conditions - Commercial Items (June 2010). (ix.)The provision at Federal Acquisition Regulation (FAR) 52.212-2 Evaluation - Commercial Items (Jan 1999) applies to this requirement. The Government will award a firm fixed price Basic Order Agreement (BOA) resulting from this solicitation to the responsible offeror(s) whose offer(s) conforming to the solicitation will be most advantageous to the Government. (x.)The provision at Federal Acquisition Regulation (FAR) 52.212-3 Offeror Representations and Certifications-Commercial Items (May 2011) applies to this requirement. The contractor shall either: 1. Return a completed copy of this provision with their quote (a copy of the provision may be attained from (http://www.arnet.gov/far); or 2.
