Description
Combined Syn-Sol SF-1449 GARBAGE REMOVAL SERVICES AT Eau Galle Recreation Area   2. Cleanliness, Safety, and Customer Complaints. The COR will receive and investigate complaints. The Contracting Officer shall make final determination of the validity of customer complaint(s) in cases of disagreement between the COR and the Contractor. Validated customer complaints shall be corrected within 24 hours of notification.   3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES 3.1 Government Furnished Facilities. 3.2 Government Furnished Supplies and Equipment. necessary keys, if applicable, to the contractor. The keys shall be used for the sole purpose of refuse collection as stated in this contract and returned to the Designated Park Official upon completion of the contract terms. If the keys are lost, there will be a fee associated with replacement. 3.3 Government Furnished Utilities. 3.4 Telephone Service. None. 3.5 Security and Fire. 3.6 Mail Service. 4. General Information 4.1 Hours of Operation. 4.2 Recreation Area Closures. 4.3 Spill Response. 4.4 Hazardous Material/ Waste Management. 4.5 Traffic Laws. 4.6 Reporting Requirements. 4.7 Safety. 4.8 Contract Manager. 4.9 Personnel. 4.11 Payment. The Contractor may be paid monthly. 5.0 APPENDICES.   APPENDIX A - Common Levels of Garbage Service Standards APPENDIX B - Site Map Dumpsters APPENDIX C - Required Reports/Forms   APPENDIX - A Common Levels of Service (CLS) Standards The government will make all publications, forms, references and report formats listed available. Publications can be accessed on-line at Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wearing of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Contractor shall provide contact information for the contract manager or alternate(s); they shall be available to be contacted during work hours. The contract manager or alternate shall be available during normal duty hours to meet on the installation within 24 hours with the government personnel designated by the Contracting Officer to discuss problem areas. The Contractor shall provide the COR telephone number(s) where surveillance results and complaints can be reported. The Contractor shall also provide to the Contracting Officer the names and phone/pager numbers of Contractor POCs for after business hours including nights, weekends, and holidays. This information will be kept updated by the Contractor whenever personnel changes occur. The contract manager or alternate shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract. All work shall be performed in accordance with the U.S. Army Corps of Engineers Safety and Health Requirements manual, Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security or safety of DOD personnel, Contractor personnel, or Campers. The Contractor and its employees shall comply with recreation area traffic regulations. The Contractor will be briefed on the Blackhawk Park Hazardous Material / Waste Management Plan at the pre-performance conference. The Contractor will be briefed on the Blackhawk Park spill response procedures at the pre-performance conference. The Contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations, in the event that it is necessary to utilize government material, equipment or personnel to clean up a Contractor caused spill, the Contractor shall be required to reimburse the government for all associated costs. Work scheduled but not accomplished because of closure will be accomplished as soon as possible. The contractor shall perform the services required under this contract during the following hours: Monday through Friday between 8:00 a.m. and 5:00 p.m. None. The Government will provide police and fire protection. None. The government shall furnish None. EM-385-1-1. http://www.e-publishing.af.mil. Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract. The Contractor shall implement those changes upon receipt from the Contracting Officer. Changes in the contract price due to supplements and amendments shall be considered under the FAR 52.212-4, Contract Terms and Conditions - Commercial Items clause. GARBAGE AND RECYCLING SERVICES: TASK / SERVICE REQUIREMENT GARBAGE DUMPSTERS PEAK SEASON 1 MAY - 31 OCTOBER ONCE PER WEEK/On-Call Monday or Tuesday GARBAGE DUMPSTERS OFF PEAK SEASON 1 NOVEMBER - 30 APRIL ON-CALL     APPENDIX - B EAU GALLE RECREATION AREA SITE MAP DUMPSTER LOCATIONS   6 Cubic Yard 2 Cubic Yard Highland Ridge Campground * 1 2 Project Office* 1 1 recycling bin             *Exact location specified by Designated Park Official at time of delivery Required Due Date Primary/Alternate POC's Upon Contract Award Customer Complaint Form As Needed   APPENDIX - C REQUIRED REPORTS AND FORMS The contractor shall ensure that all contractor personal comply with all traffic and safety precautions while on Federal property. Contractor personal shall ensure that all refuse collected remains within the collection equipment. Any refuse that spills from collection dumpsters or vehicles during unloading or transport shall be collected by contractor personal prior to exiting Federal property.   PERFORMANCE WORK STATEMENT FOR GARBAGE REMOVAL SERVICES 1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, equipment, fuel and labor necessary to ensure that garbage removal services are performed at Eau Galle Recreation area in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. The contractor shall accomplish all garbage removal tasks identified in Appendices A and B, in order to meet the requirements of this PWS and the Service Summary (SS). Garbage removal standards are established in Appendix A. A facility drawing of garbage dumpsters are established in Appendix B. All work performed by the contractor shall be performed in accordance with all applicable laws, regulations, instructions, and commercial practices. 1.1 Garbage Removal Services: 1.1.1 Dumpsters. 1.1.2 Dumpster Location 1.1.3 Peak Season Dates, Days and Hours. October 31 st each year. All dumpsters shall be emptied between the hours of 8:00 am and 5:00 pm. Dumpsters shall be serviced weekly on Mondays or Tuesdays. No service shall be performed on Mondays which are Federal Holidays (Memorial Day, Independence Day, Labor Day, etc); those services shall occur on Tuesdays. Additional services during the peak season may be requested by the government on an on-call basis, contractor shall provide requested services within 24 hours of notification. The on call services will not exceed two additional pickups during any one week and shall be limited to a maximum of 12 occurrences per period of performance (1 year). 1.1.5 Off Peak Season Dates, Days and Hours. 1.1.6 Transport and Disposal. 1.2 Contractor Provided Equipment. 1.2.1 Contractor-Furnished Vehicles. 1.2.2 Damages   1.3 Miscellaneous Requirements Report fire hazards, conditions, and items in need of repair to the COR Turn in lost and found articles to the COR Notify COR when unauthorized or suspicious persons are seen on the premises Report all acts of vandalism, larceny or pilferage to the COR The Contractor shall be responsible for notifying the Contracting Officer Representative (COR) of any apparent damage to Government buildings, equipment, vegetation, natural features or public property that are related to the work under this contract. Notification shall be in writing and furnished to the Contracting Officer Representative (COR) within 24 hours of discovery of any damage. The contractor shall provide an adequate number of vehicles to effectively fulfill the scope of this contract. In addition, the contractor shall maintain or have backup capability to provide continued service in the event primary equipment or vehicles are down for extended maintenance or repairs. Equipment failure shall not alleviate the contractor from performing any requirement contained in this contract. All vehicles shall be equipped to completely contain trash and prevent spillage/scattering of litter. In accordance with EM 385-1-1 pragraph 18. B.01 all trucks shall be operated with a properly functioning backup alarm while on government property. The Contractor shall transport and dispose of all garbage from dumpsters off government property in compliance with all state and Federal laws and regulations. Off-peak season is defined as November 1st - April 30th each year. All covered dumpsters shall be serviced on an on-call basis, contractor shall provide requested services within 24 hours of notification. Peak season is defined as May 1 st - Dumpsters shall be placed in three (3) locations as indicated on the attached drawing, Appendix B. The Contractor shall provide two (2), six (6) cubic yard and two (2), two (2) cubic yard garbage dumpsters and one (1), two (2) cubic yard recycling receptacle for Eau Galle Recreation Area. In addition, all dumpsters furnished by the Contractor must be standard manufactured items meeting the size requirement of this contract and have lids capable of closing. The government will not be responsible for the security or damage to containers or contractor's equipment.
