- HSFE70-12-Q-0078 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Submit quotations and capability information to Chase.Smith@fema.gov or fax to 202-646-3318 by 08/30/2012 at 2:00 PM Eastern Time. If you fax your quote, please email to confirm that it was received. Intended award date depends on Hurricane Isaac. Email all questions to Chase.Smith@fema.gov. This is a Request for Quotation. RFQ number: HSFE70-12-Q-0078 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-60. NAICS codes is 423450. This acquisition is a small-business set-aside. Small business size standard is 100 employees for NAICS 423450. This RFQ is posted on 8/29/2012. Contracting Office Zip Code: 20472 Award will be made to the vendor whose quote is the lowest-cost-technically-acceptable that meets the delivery schedule requirements. Please clearly verify in your quotation that all technical specifications in the following description of requirements are met. REQUIREMENT FEMA requires Consumable Medical Supplies (CMS) kits in support of Hurrican Isaac. FEMA requires five (5) complete kits for destination(s) in Louisiana, Mississippi, and/or Alabama. Please provide a price for supplying up to five (5) complete CMS kits for pick-up within twenty-four (24) hours and forty-eight (48) hours of request. Please explain how many complete CMS kits you are capable of producing within twenty-four (24) and forty-eight (48) hour periods. See attachments for Description of Work and Pricing Sheet. Be careful to quote the actual "Quantity" for each line item. "Total Quantity Per Kit" and "Total Quantity Per Kit Unit" are a measurement of the Quantity.
