Description

MUSIC THERAPY PROGRAM Provide a Music Therapy Program for all residents of the Hospice Unit at the VA Medical Center in Erie, Pennsylvania per attached statement of work. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This solicitation is issued as a request for quotation (RFQ). Submit written quotes on Solicitation VA244-12-Q-1281. This solicitation is being issued as a 100% Small Business set-aside. The associated NAICS code is 621399. The government contemplates a firm fixed price contract for this procurement. This requirement shall be completed in accordance with the Statement of Work. This combined solicitation/synopsis is for the purchase of the following commercial service: Music Therapy Program. Any/all inquiries regarding this solicitation must be submitted in writing to Cynthia Grygier, Contract Specialist at Cynthia.grygier@va.gov before 4:00 pm on 09/12/12.
