Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis solicitation W81XWH-12-T-0416 is issued as a Request for Quotation (RFQ). The solicitation document with incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. This RFQ is 100% Total Small Business Set-Aside under North American Industry Classification System (NAICS) 541611, with a size standard of $14.0 million. Offerors must indicate their business size on the Quote in response to this RFQ. The resulting contract will be a single award Firm-Fixed Price (FFP) type contract. A list of Contract Line Item Numbers (CLINS) and items, quantities including options can be found in the supplemental information included with this notice. DESCRIPTION: The U.S. Army Medical Research Acquisition Activity (USAMRAA) announces the requirement for the U.S. Army Medical Materiel Agency (USAMAA) for the International Organization for Standardization (ISO) 9001:2008 (or equivalent) Quality Management System (QMS) Recertification and Surveillance Audit Services as stated in the Performance Work Statement (PWS) found as Section C included with this notice. PROPOSAL REQUIREMENTS: Offerors shall complete the pricing using Attachment IV Price Sheet for each CLIN. The Government estimates that this service contract shall require an approximate level of effort, as outlined in Attachment IV, to accomplish the tasks identified in the PWS. Please note the Government estimate is only put in place for Offerors to ascertain the approximate or estimated level of effort; however, this is not be construed as either mandatory or necessarily the best technical approach. It is only in place as a reference to allow Offerors to better understand the general scope of this effort from the Government's perspective. The Government is seeking the best level of effort and labor mix. The Offeror shall propose the level of effort for the Key Personnel and any additional labor mix the Offeror has determined is right to accomplish the mission contained in the PWS. A Memorandum announcing the RFQ is included with this notice in addition to the following Attachments: Attachment I - Quality Assurance Surveillance Plan (QASP) Attachment II - Past Performance Questionnaire Attachment III - Key Personnel Labor Category Descriptions Attachment IV - Price Sheet Attachment V - Acronyms All quotes received in response to this RFQ must be for the entire requirement. Partial quotes will not be accepted. All Offerors are required to have an active Online Representations and Certifications Application (ORCA) record and an active Central Contractor Registration (CCR) registration. For more information, please visit the System for Award Management (SAM) website at https://www.sam.gov. Offerors not registered in SAM may result in ineligibility for contract award. Additionally, Offerors must not be listed in the Excluded Parties List System (EPLS) found at https://www.epls.gov. EPLS will soon become accessible only through SAM. Offerors found on the EPLS/SAM are barred from conducting business with the Government and shall be considered ineligible for award. The commercial Federal Acquisition Regulation (FAR) provisions at 52.212-1 Instructions to Offerors - Commercial Items (FEB 2012) and 52.212-3 Offeror Representations and Certifications - Commercial Items (APR 2012) Alternate I (APR 2011) and commercial FAR clauses 52.212-4 Contract Terms and Conditions - Commercial (FEB 2012), and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (MAY 2012) apply to this acquisition and are incorporated by reference or full text along with all other applicable FAR and Defense Acquisition Regulations System (DFARS) clauses and provisions included with this notice. PROPOSAL SUBMISSION: All questions in reference to this solicitation shall be submitted in writing via e-mail to the Contract Specialist, Evelyn Stephenson at evelyn.stephenson@amedd.army.mil by Monday, September 10, 2012 at 10:00 A.M. EST. Questions shall not be addressed by telephone and questions received after the closing date and time will not be accepted. Interested Offeror's shall submit a response (quote) based on the requirements identified in the PWS and include all required documentation via e-mail to the Contract Specialist, Evelyn Stephenson at evelyn.stephenson@amedd.army.mil no later than Friday, September 21, 2012 at 10:00 A.M. EST.
