Description
The Social Security Administration (SSA) is issuing this Request for Proposals (RFP) for the competitive acquisition of white Kraft envelope, style #535, size: 6-1/16 x 9-1/2 inches, for distribution of SSA notices. The Envelopes must meet specifications for machine inserting on the Pitney Bowes model Flexible Productivity Series Split Drive (FPS SD) inserters in accordance with the attached Statement of Work (SOW). See Attachment 1 "Addendum to 1449". We are conducting this competition exclusively amongst small business firms pursuant to the Federal Acquisition Regulation (FAR) Subpart 6.2, Full and Open Competition, After Exclusion of Sources, and Subpart 19.5, Set-Asides for Small Businesses. Our intent is to award a Requirements type contract consisting of firm-fixed price elements, spanning over a base year and four one-year option periods. We intend to have a contract in place before the end of this Fiscal Year. The North American Industry Classification System (NAICS) Code is 322232, Envelope Manufacturing, with a size standard of 500 employees. The applicable FAR references governing this acquisition include FAR Part 12, acquisition of commercial items; Part 15, contract by negotiation; Subpart 17.2, options as a special contracting method; and Part 19, small business programs. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. Offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. The Representations and Certifications are contained in "Attachment 1" of this notice. The following additional clauses are also incorporated by reference: FAR 52.217-9, Option to Extend the Term of the Contract (MAR 2000) with the following fill-ins: paragraph (a), at any time during the period of performance and 60 days; and paragraph (c), 60 months. We issue this solicitation for the envelopes under RFP Number SSA-RFP-12-1057. Interested parties must respond to the RFP in order for the Agency to consider them for any resultant contract. THE AGENCY WILL NOT MAKE HARD COPIES OF THE RFP AVAILABLE. To be considered for award, offers must meet the salient physical, functional, and performance components specified in attachment 1 of this solicitation. The contractor shall also provide a firm-fixed price and state the period of acceptance for their proposal. The Contracting Officer will evaluate offers on the basis of information furnished by the offeror. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of this solicitation in terms of lowest priced, technically acceptable offeror that represents the best value to government. EVALUATION FACTORS: The following factors shall be used to evaluate proposals: price and past performance. Responses to this solicitation are due no later than 2 p.m. EST, Wednesday, September 12, 2012. Offerors must send responses via email to Sandra.Wicks@ssa.gov. All questions regarding this RFQ must be submitted in writing to Sandra Wicks at the email address provided previously by September 10, 2012 at 2:00PM EST. NO PHONE CALLS WILL BE ACCEPTED. E-mail communications shall include company name, point of contact, e-mail address, phone and fax numbers. Offerors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov.
