Description
The Federal Bureau of Prisons, TDAT Contracting Office, is seeking proposals from sources having the ability to provide community-based outpatient transitional drug abuse treatment services, within a 10 mile radius of The United States Postal Office, 4101 S. Louise Avenue, Sioux Falls, South Dakota 57105. Performance periods under this contract will be for a one-year base period estimated to begin on December 1, 2012 with four (4) one-year option periods, with services ending (if all options years are exercised) on November 30, 2017. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. Base Year: Estimated number of inmates 24: Intake Assessment & Report - Substance Abuse (1 per inmate) for 24 inmates; Individual Units (48 per inmate) for 24 inmate; Group Units (72 per inmate) for 24 inmates; Family Units (8 per inmate) for 2 inmates; Individual Transportation (round trip for each inmate) (Est. 1,456 trips) for 28 inmates; Intake/Assessment & Report - Mental Health (1 per inmate) for 4 inmates; Mental Health Individual Units (48 per inmate) for 4 inmates; Mental Health Group Units (72 per inmate) for 4 inmates; Medication Monitoring (1 per inmate) for 4 inmates; Psychological Evaluation & Report (1 per inmate) for 4 inmates; Psychiatric Evaluation & Report (1 per inmate) for 2 inmates; psychological Testing & Report (1per inmate) for 4 inmates. Option Year Four: Estimated number of inmates 35: Intake Assessment & Report - Substance Abuse (1 per inmate) for 35 inmates; Individual Units (48 per inmate) for 35 inmate; Group Units (72 per inmate) for 35 inmates; Family Units (8 per inmate) for 4 inmates; Individual Transportation (round trip for each inmate) (Est. 1,456 trips) for 39 inmates; Intake/Assessment & Report - Mental Health (1 per inmate) for 4 inmates; Mental Health Individual Units (48 per inmate) for 4 inmates; Mental Health Group Units (72 per inmate) for 4 inmates; Medication Monitoring (1 per inmate) for 5 inmates; Psychological Evaluation & Report (1 per inmate) for 5 inmates; Psychiatric Evaluation & Report (1 per inmate) for 2 inmates; psychological Testing & Report (1per inmate) for 5 inmates. The services shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work dated April, 2011 - Master's Not Required. The request for proposals will be available on or after September 24, 2012 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the proposal will not be available. The site provides downloading instructions. All future information about this acquisition, including proposal amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about October 4, 2012. Faith-Based and Community -Based Organizations can submit offers/bids/proposals equally with other organizations for contracts for which they are eligible. The BOP anticipates awarding a firm-fixed price contract. All responsible small business concerns may submit a proposal which will be considered by this agency.
