- REPORT REPORT COPIES OF QUESTIONS AND ANSWERS AMENDMENT#1: THE FOLLOWING DOCUMENTS HAVE BEEN ATTACHED TO THE SUBJECT SOLICITATION: COPIES OF ALL QUESTIONS AND ANSWERS LIST OF CONFIGURATIONS RESPONSE DATE REMAINS UNCHANGED. COMBINED SYNOPSIS/SOLICITATION AVAYA TELEPHONE SWITCH UPGRADE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. a. Solicitation Number HQ003021390001000 is being issued as a Request for Quotation (RFQ) and incorporates the following with the word "quoter" substituted for "offeror". b. Provisions and clauses are those in effect through both Federal Acquisition Circular 2005-60, effective 27 August 2012 and DFARS Change Notice 20120724. c. This procurement is 100% small business set-aside, associated with NAICS Code 334210, and small business size standard of 1000 EMP. d. Description: Note: THIS ITEM IS BRAND NAME ONLY. SEE THE ATTACHED STATEMENT OF WORK AND JUSTIFICATION. CLIN 0001: Avaya Aura Communication Upgrade in accordance with the attached Statement of Work and Bill of Material. e. Delivery and acceptance will be made at: AFN-BC ENGINEERING 23755 Z STREET RIVERSIDE, CA 92518 f. FAR Provision 52.212-1 (Instructions to Offerors - Commercial Items) (FEB 2012) applies to this acquisition. Para (k) requires current registration in CCR at http://www.ccr.gov. g. FAR Provision 52.212-2 (Evaluation - Commercial Items) (JAN 1999) with the following insertion in Para (a) "lowest priced technically acceptable with acceptable past performance." Technical evaluation will be determined in accordance with "Description" paragraph (d) above. h.
