Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Mission and Installation Contracting Command (MICC) - Fort Sam Houston intends to award a firm fixed-price contract on a sole source basis in accordance with FAR 6.302-1(a)(2)(ii)(A) to Parata Systems, LLC, 2600 Meridian Parkway, Suite 100, Durham, North Carolina 27713-2203, for maintenance services of the Parata Pharmacy 2000 system in support of the patients empanelled to the Schertz Medical Home located in San Antonio, Texas. The services are commercial within the meaning of FAR Clause 52.212-1. The purchase order will be issued on or about 21 September 2012 with a period of performance of one year and two, one-year options through September 2015. Estimated initial cost of services is $30,392.00 and the yearly cost for each subsequent year at $30,392.00 per year for a total cost of $91,176.00. The system supports both the initial prescription and prescription refill services performed at the Schertz Medical Home.
