- Description
- (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 12-236-SOL-00050 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-61. (iv) The North American Industry Classification System (NAICS) code is 611710. The Indian Health Service (IHS), Division of Acquisition Policy intends to award a firm fixed price purchase order to Reach Out and Read (ROR) National Center on the basis of other than full and open competition. (v) The Contract Line Item (CLIN) is as shown below: CLIN 0001 (POP: September 27, 2012 ~ Sep 26, 2013) - 1 year ROR Support Services for Indian health Service/Tribal/Urban (I/T/U) health care facilities. Unit: 1 Year - $__________ (vi) The Government intends to negotiate and award an order to ROR, 56 Roland Street, STE 100D, Boston, MA 02129, to obtain ROR provided site based training, technical assistance, and quality improvement for Indian Health Service/Tribal/Urban (I/T/U) health care facilities. ROR is a national literacy program conducted in health care settings, hospitals, clinics and public health nurse settings for infants, toddlers, young children and their parents. I/T/U clinics provide 75 percent of health care for the AI /AN children. Currently, 200 out of 400 I/T/U health facilities have already implemented ROR program and the effort cannot feasibly be competitively sourced without an unacceptable duplication of effort if awarded to other than the incumbent. The proposed additional work will implement the ROR program to the rest of the I/T/U facilities and is a logical follow on effort that ROR will use the information and knowledge developed from the past experience in providing the services to AI/AN population. Interested parties may identify their interest and capability to respond to the requirement or submit quotations. This notice of intent is not a request for competitive quotations however; all responses received within five (5) days from the date of publication of this synopsis will be considered by the Government. (vii) n/a (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Feb 2012), applies to this acquisition and a statement regarding any addenda to the provision (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The acceptability of the Technical Quotation and Price will be evaluated. (i) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items (Apr 2012), with its offeror. (ii) FAR 52.212-4 Contract Terms and Conditions - Commercial Items (Feb 2012) applies to this acquisition.
