Description
FIXED PRICE STATEMENT OF WORK FOR PEER SUPPORT TO DEPARTMENT OF VETERANS AFFAIRS August 14, 2012 This SOW, including cover page, consists of 7 pages 1.0DESCRIPTION OF SERVICE This statement of work (SOW) describes the tasks required of the contract to support the Department of Veterans Affairs by providing rehabilitative peer support services as described in Section 2.0, Requirements, in compliance with applicable documents listed in paragraph 1.3. Contractor(s) shall be responsible for supporting their established Area of Responsibility (AOR). 1.1BACKGROUND This SOW specifies services for the work provided under Title 38, Code of Federal Regulations, Chapter 31. The peer support service will be provided across the entire state of Michigan under the Department of Veterans Affairs Detroit Regional Office geographic purview. The Department of Veterans Affairs (VA) has authority, pursuant to Title 38 USC Chapters 18, 31, 35 and 36, to provide all services and assistance necessary to entitled Veterans with service-connected disabilities to obtain and maintain suitable employment and, if not employable, achieve independence in daily living to the maximum extent feasible. In the discharge of this responsibility, the Vocational Rehabilitation and Employment Service Division (VR&E) within each VBA Regional Office's (RO) jurisdiction undertakes an initial evaluation of the Veteran to determine his or her entitlement for these services and assistance and develops, in cooperation with the Veteran, an Individualized Written Plan of Services. Rehabilitation plans include: Individualized Written Rehabilitation Plans (IWRP) that outline training and job placement services needed to achieve employment goals; employment placement under an Individualized Employment Assistance Plan (IEAP); Individualized Extended Evaluation Plan (IEEP) to determine feasibility for employment; or if employment is not possible, services to enhance the Veteran's independence in daily living under an Individualized Independent Living Plan (IILP). 1.2. The Veterans Benefits Administration (VBA). Department of Veterans Affairs (VA) is charged with assistance to provide high quality services to veterans with service-connected disabilities. In order to assure services in close proximity to veterans and to provide timely services when staff turnover occurs or caseload size inhibits the VRC's ability. 1.2SCOPE VR&E services. Specialized services may also be required to complement services typically provided by the VA staff. The VetSuccess program administered through the Department of Veterans Affairs assists veterans preparing for, finding, and retaining suitable employment for veterans with service-connected disabilities so severe that they require Special Rehab Services. VetSuccess offers services to veterans to improve their ability to live as independently as possible, or to improve their potential to obtain and maintain suitable employment. Required services may include peer support. Peer support is a system of one on one and/or group pairing of the veteran with an individual(s) having a certain disability, another veteran(s), or an individual(s) with a substantial history of working in the field of disability. This pairing allows the veteran to receive help founded on key principles of respect, shared responsibility, and mutual agreement of what is beneficial. Peer support is not based on psychiatric models and diagnostic criteria. Peer support would enable understanding of one another's situation empathically through the shared experience of emotional and psychological pain. Contractor(s) would help facilitate this need along with providing appropriate reporting to the VA from each authorized referral. When people find affiliation with others whom they feel are "like" them, they feel a connection. This connection, or affiliation, is a deep, holistic understanding based on mutual experience where people are able to "be" with each other without the constraints of traditional (expert/patient) relationships. Further, as trust in the relationship builds, both people are able to respectfully challenge each other when they find themselves re-enacting old roles. This allows members of the peer community to try out new behaviors with one another and move beyond previously held self-concepts built on disability, diagnosis, and trauma worldview. Peer support will lead to the veteran becoming fully integrated into their community, while establishing a support network that will be sustainable for the veteran. Community integration entails accessing community resources and increasing social comfort in public situations. Peer support often involves the peer support specialist moving through the community with the veteran as they practice their newly obtained special rehabilitation skills. APLLICABLE DOCUMENTS 1.2.1Reference Documents. The Contractor(s) shall have access to and use of all the latest documents relating to Department of Veterans Affairs involvement, as needed by the services needed, unless otherwise directed by the Government. 2.0REQUIREMENTS The Contractor(s) will provide peer support services as required to achieve the objectives of the referral or as prescribed by the written rehabilitation plan provided by the VA, to include supportive information necessary in completing frequent and ongoing services to the veteran. The Contractor(s) providing peer support shall: 1)Establish rapport with the veteran and maintain active contact with the veteran via face-to-face meeting. The purpose of the support is to continually assess all aspects of the Ch.31 services provided. 2)Obtain information needed to document the veteran's progress, including limitations, barriers and overall situation, etc. 2.1Peer Support Planning and Documentation: 2.1.1.The Contractor(s) shall assume cognizance of the peer support Government developed baseline documentation theory, implementing an effective system support strategy for within their respective AOR. 2.1. The Contractor(s) shall be responsible to identify program issues and facilitate effective efforts with other entities involved in the servicing of peer support activities with the VA, outside agencies, and industry partners. 2.1.3 The Contractor(s) shall develop peer support reports to submit to originating referral or requestor. 3.0KEY PERSONNEL 3.1.1. Key personnel listed below are considered essential by the Government and to the work being performed under the Peer Support efforts under the contract. The key personnel assigned to or utilized by the Contractor in the performance must be accessible to the Government for services under the contract. Contractor and staff must be of equal or better qualifications that will be approved by the Contracting Officer's Representative (COR). The Contractor and/or Key Personnel shall be insured, licensed and/or certified if required by the state or local statute for all work assigned. The Contractor shall supply proof of state required insurance, license and/or certification to the Contracting Officer within 30 days after notice of award. The following are the list of Key Personnel: a. Program Manager b. All Involved Counselors 3.1.2. The Government reserves the right to examine, and approve or disapprove the Contractor's staff based on information provided in credentials and/or résumés. The COR, at the RO will notify the Contractor of acceptance of newly proposed employees within 10 days of receipt of credentials, diplomas, and résumés. 3.1.3. Only those individuals that meet the qualifications of the contract, and have been approved by the COR, at the RO shall perform services under the contract. Under no circumstances will interns be approved to provide professional services under the contract. 3.1QUALIFICATION REQUIREMENTS FOR KEY PERSONNEL 3.2.1. Contract Administrator/Program Manager: Contract Administrator/Program Manager must have one (1) year experience in Technical Vocational Rehabilitation and management of rehabilitation services. They must possess communication/office skills commensurate with the job. 3.2.2.All Counselors: Must meet the qualifications listed in any item 1, 2 or 3 as described below, having a minimum of the following: 1. A Masters Degree in Rehabilitation Counseling, Social Work, and Counseling Psychology or a closely related field from an accredited college or university. 2. Bachelor's Degree in Psychology, Social Work, or closely related from accredited colleges or universities; AND IN ADDITION TO THIS DEGREE, POSSESS: a.Possess three or more years of work experience in the related industry field. 3. Registered Nurse (RN) or a licensed Occupational Therapist (OT) who has graduated from a nursing program at an acredited college or university and has passed a national licensing exam or a licensed vocational nurse (LVN); AND IN ADDITION TO THIS DEGREE, POSSESS: a.Possess three or more years of work experience in the related industry field. 3.2.3Contractors must submit resumes, transcript and copy of CRC for each counselor working on the Peer Support contract at time of award. These credentials are required for all counselors working in any service group. Counselors, in the performance of services under the contract, shall use standard counseling principles and procedures generally recognized within the counseling profession by the Commission on Rehabilitation Counselor Certification (CRC) at http://www.crccertification.com. 4.0GEOGRAPHICAL SERVICE AREAS FOR PEER SUPPORT 4.1. Requests for peer support services shall be referred by VBA VRC(s) to Contractor(s) from among those Veterans living in the Detroit Regional Office jurisdiction and within the scope of Ch.31. 5.0GOVERNMENT FURNISHED EQUIPMENT/PROPERTY The Government shall not provide workspace, computers, telephones, and any other device or facility for contractor personnel performing work under this peer support contract.
