Description
STATEMENT OF WORK MMODAL/LANIER DICTATION SYSTEM SCOPE: This service contract shall include a request for support to cover the MedQuist Dictation System, Model #2400NT, Serial #10600015 located at the Veterans Affairs Medical Center (VAMC) Washington, DC. The vendor will provide all labor, tools, travel cost, equipment, and parts required to perform preventive maintenance (PM), repair, installation, and software updates on equipment (hardware and software) covered by the contract. All equipment shall be maintained in optimal operating condition per the model specific manufacturer's service manual. Only original equipment manufacturer (MedQuist) parts, manuals, and schematic will be utilized to perform the service. These must be available on site at time of service. All defective parts replaced become the property of the vendor. HOURS OF COVERAGE: Service will be provided on a 24/7 basis Monday - Sunday. Service includes onsite service as well as phone assistance. The vendor must be knowledgeable of when it is more beneficial to provide phone assistance or onsite service. When Service is rendered after 5pm Monday-Friday, on weekends, and on holidays, arrangements will be made for access to the Radiology Service through the Contracting Technical Representative (COTR) or designee. Each trip to the DC VAMC requires stopping at the Security Office to obtain a vendor's badge. RESPONSE TIME: Contractor shall provide a phone call back by a qualified technician within thirty minutes of a service request and on-site response by a qualified technician within two (2) hours of an original request for service. Contractor will report to the Security Office for badging first and then Radiology Service upon arrival during normal business hours. QUALIFICATION OF VENDOR: Vendor personnel must be trained and certified to repair the specific equipment listed in the contract and must submit a certificate of training to the COTR prior to commencing work on this contract. Furthermore, vendor must be knowledgeable of MedQuist software capabilities and must be able to advise Radiology Service of when updates should be implemented. QUALITY ASSURANCE (QA) Services provided by the vendor will ensure that the system operates optimally. This means that workflow should not be interrupted for more than eight hours (8 hours) any day of the week due to system malfunction. Needed repairs must be provided on the same day the service call is placed. Furthermore, it is expected that the system will be properly repaired during the first service call. Any QA standards not met, as outlined above, may be reason for termination of the contract by the VA. INSTALLATIONS: Initial installations of hardware and software shall be provided to the VA for dictation equipment purchased from MedQuist. The company shall inform the VA of any price changes to contract due to new installations. UPDATES: Vendor will provide all software and hardware updates as indicated. Software and hardware updates a limited to those items, which are commercially available from the MedQuist Healthcare, LLC. In no instances should parts from other vendors be used in repair, as this may cause the system to malfunction. LOANERS: In the event hardware must be taken off station to be repaired, a suitable loaner with similar capabilities will be loaned to Radiology Service until the original equipment can be properly repaired. This service will be provided at no additional charge, unless Radiology Service damages or misplaces the loaner equipment. DOCUMENTATION REQUIREMENTS: Field Service Reports shall be provided at the time of the service call and must be signed by the Contracting Technical Representative (COTR) or designee and filed with the Radiology Service. Field Service Reports shall detail the work performed,i.e. repair, performance, procedures, calibrations, parts and labor provided. LOCATION OF AUTHORIZED BRANCH OR SERVICE DEPARTMENT: The vendor to provide specific name, address and phone number to be used for emergent repairs and scheduled repairs. PAYMENTS: Invoices for payment of preventive maintenance and service maintenance agreement should be issued quarterly of the beginning of the contract year. Pricing for software updates and replacements not included in the contract will be added as needed to the original contract price. Payment for these items will be issued 30 days after receipt of invoices. OPTION YEARS: The terms of the contract shall be set forth for one base year and four option years. COMPLIANCE WITH REGULATIONS: The Contractor will abide by all Medical Center and VA regulations that may affect his/her scope work. Such policies include, but are not limited to mandatory educational training, background investigation checks, and conflict of interest guidelines. Radiology Service's COTR or designated alternate will provide the contractor with Medical Center Policies that outline these issues for his/her review.
