- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA241-12-R-3059 and is issued as a Request For Quotation (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-60. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 with a small business size standard of 100 employees. This requirement is a Small Business set-aside and only qualified offerors may submit quotations. FOB Destination. Delivery and installation shall be to multiple shipping locations: VA Jacksonville Outpatient Clinic, 1536 N. Jefferson St., Jacksonville, Florida and VA Specialty Clinic, 6900 Southpoint Drive, Jacksonville, Florida. Quotations submitted MUST offer a 60 day acceptance period. Offeror must be registered in the System for Award Management (SAM) database before an award can be made to them. If the offeror is not registered in the SAM, it may do so through the SAM website at https://www.sam.gov/portal/public/SAM/. A single site visit will be schedule for interested parties. The VHA NCO 08 - 573 - Gainesville, FL requires the following: STATEMENT OF WORK: Offeror shall furnish, labor, materials, and equipment necessary for the installation of two (2) Solo-Step Patient Lift Systems. New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Location #1: 6900 Southpoint Drive, Jacksonville, FL 32216 One (1) 107'-0" long U-Shape Solo-Step Patient Lift Support System consisting of: Upper rails fastened to deck located approx. 12'-0" AFF, by ½" x 5" concrete mechanical anchors; Vertical supports and diagonal bracing; Finish rails located at the 8'-7" AFF; This Solo-Step System has Multiple Turns measuring 22'-0" by 70'-0" by 15'-0" designed to support a 800 lb. vertical load at any one point along the finish rails with less than 1/16" deflection; and Hardware is to be furnished in an electro-galvanized finish. Location #2: VA Outpatient Clinic, 1536 N. Jefferson Street Jacksonville, FL 32209-6525 One (1) 45'-0" long straight run Solo-Step Patient Lift Support System: Upper rails fastened to deck located approx. 13'-0" AFF, by ½" x 5" concrete mechanical anchors; Vertical supports and diagonal bracing; Finish rails located at the 9'-0"" AFF; This Solo-Step System is one continuous 45'-0" run with no turns; Support is designed to support a 800 lb. vertical load at any one point along the finish rails with less than 1/16" deflection; and Hardware is to be furnished in an electro-galvanized finish. QUALIFICATIONS/CERTIFICATIONS: The contractor shall be qualified and have the required levels of professional and technical experience to perform the work. SAFETY REGULATIONS: Contractor shall comply with VA Safety Directives. All applicable safety regulations shall be observed during all work on this contract. The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. The contractor will provide all the necessary signage/barricades where required. SCHEDULING AND HOURS OF WORK: Monday thru Friday 8am to 4:30pm, and all Federal Holidays. Work shall be scheduled for proper execution to completion of this contract. The contractor shall perform this work during normal business hours, Monday through Friday between 8:00am and 4:30pm. The 10 Holidays observed by the Federal Government are New Years Day, Presidents Day, Martin Luther King Day, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, or any other day specifically declared by the President of the United States to be a national holiday. Requests For Information (RFI): Interested offerors may submit requests for information concerning the solicitation no later than September 24, 2012 by 12 noon local time.
