- THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE ONLY FOR: GENERAL CONTRACTOR WITH DESIGN BUILD CAPABILITIES, KAYENTA HEALTH CENTER STAFF QUARTERS, KAYENTA, AZ. THIS IS NOT A REQUEST FOR PROPOSAL. THERE WILL NOT BE A SOLICITATION, SPECIFICATIONS, OR DRAWINGS AVAILABLE AT THIS TIME. EACH RESPONSE SHALL BE REVIEWED BY THE CONTRACTING OFFICER FOR THE PURPOSE OF DETERMINING WHETHER THERE EXISTS REASONABLE EXPECTATION OF OBTAINING A SUFFICIENT NUMBER OF OFFERS FROM RESPONSIBLE SMALL BUSINESSES THAT ARE COMPETITIVE IN TERMS OF CAPABILITY, QUALITY, BONDING CAPACITY AND DELIVERY. A DETERMINATION NOT TO COMPETE THE PROPOSED PROCUREMENT BASED UPON RESPONSES TO THIS NOTICE IS SOLELY WITHIN THE DISCRETION OF THE GOVERNMENT. ALL INFORMATION FURNISHED TO THE GOVERNMENT IN RESPONSE TO THIS NOTICE WILL BE USED FOR REVIEW PURPOSES ONLY. A market survey is being conducted to determine if there interested Small business, HUBZone Small Business, 8(a) Small Business, Woman Owned Small Business, Small Disadvantaged Business, or Service Disabled Veteran Owned Small Business, with current relevant qualifications, experience, personnel and capability to perform the following proposed design/build contract: DESIGN AND CONSTRUCT (DESIGN-BUILD): 129 units of housing to support the operation of the new Indian Health Service hospital currently under construction in Kayenta, AZ. Housing will be collocated with the hospital on a single plot of land with a common approach and shared circulating roadway. Housing units will be composed of a mix of occupancies and configurations to best utilize the site and to support recruitment and retention of non-local medical staff. Emphasis will be placed on developing the housing to incorporate specific sustainability goals, operational maintainability, community features, and how the site layout, massing, and organization enhances the public and staff functions of the hospital environment. Project phasing is anticipated. The initial phase will complete all design, fully develop all site utilities, roads, paths, sidewalks, lighting, support structures, and other common features along with some number of initial housing units. NAICS Code is 236115. Size Class $33.5 Million. Estimated Cost Range Design/Build Construction - $30 to $50 Million. WHAT AND HOW TO SUBMIT -It is requested that interested small businesses submit to the contracting officer: All interested Small business, HUBZone Small Business, 8(a) Small Business, Service Disabled Veteran Owned Small Business, Women Owned Small Business, or Small Disadvantaged Business. Contractors should respond to this office in writing by EMAIL ONLY on or BEFORE 21, OCTOBER 2012 @ 11:00 PM local Seattle, WA time. SEE BELOW IN THIS NOTICE FOR PAGE LIMITATIONS, EMAIL ADDRESS AND WHAT TO TYPE IN EMAIL SUBJECT LINE. A Written Letter of Interest and a brief capabilities statement package (no more than 10 pages in length, single spaced, 11 point font minimum) demonstrating ability to perform the requested services. PAGES IN EXCESS OF THE FIRST 10 PAGES (exclusive of bonding letter) WILL NOT BE CONSIDERED. The documentation shall address/include as a minimum, the following: (1)Company name and profile, office location(s), number of employees, DUNS number, and CAGE Code, and identification as a small business, HUBZone Small Business, 8(a) Small Business, Women Owned Small Business, Service Disabled Veteran Owned Small Business, Small Disadvantaged Business. (2)Letter from Bonding Company certifying that the small business firm has available bonding capacity of at least $40 million. (3)Past Experience as a PRIME CONTRACTOR with DESIGN-BUILD Contracts similar in scope, scale, dollar value and purposed for residential staff-housing campuses supporting government operations/facilities in remote locales. Provide descriptions of your firm's past experience on Design-Build projects with greater than 80% construction complete or those projects completed within the last six years which are similar to this project in size-scale, scope, and dollar value ($30-$50 million or greater): For each project submitted, include: Name of project and location, description of scope and scale of project, dollar value, whether or not the project was design/build, percentage/portion of work that was self-performed.
