Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The Request for Proposal number is RFQ VA-249-13-Q-0028. Proposals are due no later than 12:00 pm EST on 22 October 2012, and shall be sent via email to PHON.PHASAVATH1@va.gov. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a quotation for the PATHOLOGY LAB OUTSOURCE TESTING SERVICE for the VAMC Memphis, Tennessee. This acquisition will be processed under Commercial Item Procedures with an approximate delivery period of 01 October through 30 September 2013 and two option years. This procurement is solicited on an unrestricted basis. Award will be made to the lowest price technically acceptable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43, subject to the availability of funds, and solicited on an unrestricted basis for NAICS 621511, with a size standard of $13.5 in millions of dollars. All responsible sources may submit a proposal which shall be considered by the agency. Provisions and clauses in effect through Federal Acquisition Circular 2005-36 are incorporated. The FMC has a requirement for pathology lab outsource testing in accordance with the following statement of work. Quantities are estimated. Orders will be placed on an as needed basis and delivered to Department of Veterans Affairs VAMC Memphis at 1030 Jefferson Avenue Memphis, TN 38104. Payment will be made for actual quantities ordered and delivered to the Government using electronic Funds Transfer (EFT). Descriptions and estimated quantities are indicated.
