- Combined Synopsis/Solicitation Notice This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The combined synopsis/solicitation notice does not require a minimum 15-day delay between notice publication and solicitation release, thus allowing both buyers and vendors to save time publishing and responding to a "commercial items" opportunity more quickly. This solicitation is issued under VA777-13-R-0007. PCO East-St. Louis Contracting Department intends to award a sole source fixed price to Commission on Accreditation of Rehabilitation Facilities (CARF), 6951 E. Southpoint Rd., Tucson, AZ 85756-9407 under the authority of FAR 6.302-1 "Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements." A Veterans Health Administration (VHA) Handbook 1170.01 Accreditation of Veterans Health Administration Rehabilitation Programs, defines the scope of VHA's relationship to CARF in accrediting VHA rehabilitation programs, and requires VA's use of CARF. No solicitation or Request for Proposal (RFP) is available for this procurement. Nevertheless, any firm that believes it is capable of meeting the requirements described herein may submit a capability statement; which, if received by the closing date of this announcement will be considered. All responses are due by email to Anthony.Balestreri@va.gov by October 26, 2012 at 5:00 PM EST. TASKS TO BE ACCOMPLISHED: The Contractor shall provide the necessary resources to the Department of Veterans Affairs (VA), Veterans Health Administration (VHA) health care organizations to maintain their accredited state as described herein. The Contractor shall also provide the following: a. Accreditation Services: The contractor shall provide a full range of accreditation services to include accreditation-related services that are offered and conducted on a continuous basis. There are currently 147 VA facilities that will be accredited (127 Hospitals, thirteen (13) Ambulatory, and seven (7) CMOPS). Each facility may have several surveyable programs (e.g., long term care, home care, ambulatory, behavioral health, hospital, OTP, etc). The Contractor shall provide: 1. On-site surveys of VA facilities, full unannounced currently conducted on a triennial cycle and initial full conducted when a new facility. Off cycle on-site other surveys (e.g., quality monitoring event, clarification, follow up survey and new program extension) will be conducted on an as needed basis and as defined by the Contractor's published standards. 2. Standards interpretation services as necessary to include education and clarification regarding standards and the accreditation process. This may be done via emails, conference calls). 3. Continuous access to and technical support of electronic tools (e.g., application, periodic performance review, accreditation manager plus) by all organizations. 4. A dedicated account representative and a dedicated backup account representative for the VA Health System to ensure consistency and continuity throughout the process. Primary account representative should observe at least one survey per year to better understand the issues that arise during survey and the VA system. Account representative will participate in on monthly conference call, via teleconference using the VA VANTS system, where the field is updated on changes Contractor's survey process, activities or standards. The call will be audio taped and posted to the QSV website. 5. Notice of all on site surveys to the COTR/VA's Corporate Liaison within 30 minutes prior to the survey team's arrival at a VA facility. Notice to COTR via email when reports, including clarification reports are posted. 6. The Contractor shall avoid all Federal holidays, including Veterans Day, during survey scheduling. The Contractor shall avoid all known OIG CAP review days during survey scheduling. The VA will provide the dates of all known OIG CAP review to TJC every six months (January and June). 7. Intracycle Monitoring with touch points: On-site focused surveys of VA facilities conducted at the request of the VA QSV program office. These will be conducted on an as needed basis and can only be initiated only by the program office. At touch point 1 (within year one after "Survey"), telephone consultation. At that point, develop plan for touch point 2 ("Lower" performers may benefit from on-site consultation in the second year of survey. b. Corporate Orientation and Summation: The services provided shall include an annual corporate orientation and summation. 1.Orientation: The orientation allows the Contractor the opportunity to be oriented by VA staff to VA program, services and processes. The Annual orientation will take place in January or February of the contract base period or option year. The Contractor's survey team leaders will attend the meeting via teleconference using the VA VANTS system. The teleconference will be recorded and the audio of the call and handouts posted on the QSV intranet site. The Tri-annual CMOP Orientation. The orientation allows the VA the opportunity to orient Contractor team leaders to VA systems related to CMOPS. The Contractor's VA CMOP survey team leaders will attend the orientation. The teleconference will be recorded and the audio and handouts posted on the QSV intranet site. 2.Summation: The summation allows the VA the opportunity to receive a summary report of facilities' performance on the surveys and what opportunities for improvement may exist. At the Quarterly summation survey data will be reviewed, presentation shall include trends and, as appropriate, any proposed actions or other information. The data presented in the summation will include internal and external comparative data. In January the quarterly summation will include previous year's data. In April the quarterly summation will include January, February and March data. In July the quarterly summation will include January, February, March, April, May and June data. In October the quarterly summation will include January, February, March, April, May, June, July, August and September data. The Contractor's presenter will attend the meeting via teleconference using the VA VANTS system. The teleconference will be recorded and the audio of the call and handouts will be posted on the QSV intranet site. c. Manuals/Publications: The contractor shall provide manuals/publications in both print and automated format, in accordance with current published price list and agreed upon discount. 1.All VA facilities will receive manuals and manual updates for the programs they are surveyed under. 2.VA Central Office, Office of Quality, Safety and Value, Director of Division External Accreditation Program and Services will receive manuals and manual updates for the programs VA facilities are surveyed under. 3.VA Central Office, Office of Quality, Safety and Value, Director of Division External Accreditation Program and Services will receive PDF copies of manuals and manual updates for the programs VA facilities are surveyed under, once received via email will be posted on Office of Quality, Safety and Value internal website for viewing by VA employees. 4.Portable Comprehensive Accreditation Manual for Hospitals (CAMH) and CAMH manual updates will be provided to the following: One (1) to each of the twenty-one (21) Veterans Integrated Service Network (VISN) Quality Management Officers. VA will provide listing of the 21 Quality Management Officers and update as necessary; One (2) to VA Central Office, Office of Quality, Safety and Value, Director, Director of Division External Accreditation Program and Services; and One (1) to the VA Central Office, Office of the Deputy Undersecretary for Health for Operations and Management. d. Accreditation Manager Plus (AMP) 1. VA facilities and COTR will receive access to extranet version of the Accreditation Manager Plus (AMP) tool that includes electronic manuals and manual updates for the programs they are surveyed under including any software upgrades to enhance usefulness of the tool such as the tracer module. 2. Electronic manuals from the AMP will be posted on Office of Quality, Safety and Value internal website for viewing by VA employees. e. Database: The contractor shall provide a Microsoft excel spread sheet monthly to the COTR for VA Site name/HCO, type/initial or clarification report, survey dates, standard(s) cited, and observation(s). f. Laboratory Surveys: The contractor shall provide Laboratory surveys, which are based on the types of specialties and the number of sites that include these specialties. There are 14 organizations with laboratories currently accredited with the Contractor. All fourteen (14) accredited organizations with laboratories will be billed annual fees.
