RENOVATE SPACE FOR PRIMARY CARE IV, BLDG 160, JAMES H. QUILLEN VA MEDICAL CENTER, MOUNTAIN HOME, TN PROJECT No. 621-12-162 VA249-13-R-0050 Phase I Estimated Solicitation Date: 5 November 2012 Phase I Estimated Response Date: 5 December 2012 Estimated Award Date: 5 February 2013 The Department of Veterans Affairs, Veterans Administration Medical Center is preparing to issue a solicitation(Request For Proposals-RFP) for a two-phase design-build contract to renovate approximately 7,700 square feet of existing space on the second floor of Building 160 at the James H. Quillen VA Medical Center into outpatient clinical exam and procedure rooms, medical staff support spaces, administrative staff spaces, conference and lounge areas, waiting and reception areas, multipurpose and classroom spaces, restrooms, and corridor and circulation areas in accordance with VA Design and Space Planning Criteria and VA Master Specifications. SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) NAICS: 236220 Commercial and Institutional Building Construction SMALL BUSINESS SIZE STANDARD: $33.5 million PROJECT MAGNITUDE: $750,000 to $1,000,000. PROJECT LOCATION: James H. Quillen VA Medical Center, Mountain Home, TN PROJECT OVERVIEW: The project will require the selected contractor to provide all technical design and construction services, supervision, labor, materials, tools and equipment necessary for a complete engineering and construction project to renovate into exam and administrative space for use in implementation of Primary Care Medical Home Model initiative. The Two-Phase Design-Build solicitation and evaluation process shall be conducted in accordance with Federal Acquisition Regulation (FAR) Subpart 36.3 (Two-Phase Design-Build Selection Procedures). The issued RFP shall set forth the requirements for responding to Phase I of the solicitation, as well as all technical evaluation factors and sub-factors for both Phase I and Phase II. At the closing date of the Phase I Solicitation, the government shall evaluate responsive proposals received in response to Phase I of the solicitation to short-list no more than three (3) contractors to Phase II. Only those successful firms selected as a result of Phase I evaluations will be requested to submit technical and pricing proposals for Phase II. Phase II proposals shall be evaluated in accordance with Phase II Evaluation Process.
